PurposeThrough retrospective study of digital surgical template guided implants dentistry cases and compared with the related indicators of conventional implant methods cases.The aim is to provide reference and guidance for clinical application of computer-aided surgical guided template and prevention and reduction of complications.MethodsRetrieve the medical records of digital surgical template guided implants from January 2011 to December 2017 and the medical records of conventional implant methods cases from January 2009 December 2014,which was finished in the oral implant department of Yantai Stomatological Hospital.Count the implant survival rate,the implant success rate,clinical complications and the health status of soft and hard tissue around the implant,and to compare related research projects with the conventional methods and analyze the related factors.Results(1)The total number of implants included in the Department of Implant Restoration in Yantai Stomatological Hospital was 3680,including 1737 males and 1943 females,with a total of 6631 implants.There were 133 patients with digital implant guided implant,and 408 implants were implanted,65 males and 68 females.A total of 3557 patients,1672 males and 1885 females,6223 implants were implanted with conventional methods,(2)Failure of 5 implants in digital guide plate guided implant cases,the survival rate is 98.7%;the success rate is 94.9%.Compared with the conventional method(the survival rate 98.9%;success rate 91.2%),the survival rate was not significantly different,but the success rate was higher than that of the conventional method(P=0.01).(3)There were no complications such as nerve injury,maxillary sinus perforation,abnormal bleeding were founded in digital surgical template guided cases.The incidence of early postoperative complications,such as bleeding and infection,was less than that of conventional surgical procedures(P = 0.006,P = 0.02).The incidences of postoperative complications of dental implant with bone grafting was higher than flap surgical method and flapless surgical method(P=0.008,P=0.017).(4)One year after permanent prosthesis guided by computer-aided surgical template The proximal and the distal average bone resorption was 0.96 ±0.52 mm and 1.07 ±0;61 mm respectively.The total average marginal bone resorption was 0.87 ±0.45 mm.There was significant difference in bone resorption between food impaction and non-food impaction implants(P=0.36).(5)The average plaque index of implants was 1.25±1.06.25.7%of implants were not bleeding and 39.2%of implants had a bleeding index of 1.Gingival sulcus linear bleeding was about 26.7%,8.4%of implants bleeding index were 3,for severe bleeding.The probing depth measurement is range from 0-6 mm,the average probing depth is 2.64±1.09 mm.87%implants probing depth≤3 mm,13%implants probing depth>3 mm.Results(1)The clinical effect of digital template guided implant technique is superior to that of conventional free hand implantation technique.(2)In the application of digital template guided implant technique,we should pay attention to every link of guide plate design,manufacture and application,and effectively control the influence of guide template precision error on the effect of implant operation.
