ObjectivesThe aim of this study was to investigate missed nursing care in city level hospitals, secondary or tertiary,in the capital city to identify most frequently missed nursing care in clinical practice and main influencing factors including nursing job satisfaction. These shall contribute to the prevention of missed nursing care and the improvement of patient safety and quality of nursing care.MethodsA questionnaire survey was conducted among ward nurses in eligible hospitals, secondary or tertiary, through the Wenjuanxing platform September 6 to October 15,2018.Data was collected using the general information sheet, the MISSCARE Survey and the McCloskey/Mueller Satisfaction Scale(MMSS).The SPSS 22.0 was to perform the descriptive and frequency analysis, while Chi-square test.Fisher's exact test,Mann-Whitney U test or Kruskal-Wallis H test were employed to examine the distribution difference across varied groups.Pearson correlation was conducted to examine the relationship between missed nursing care and nursing job satisfaction.Binary logistic regression analysis was used to analyze the risk of missed nursing care and predictive factors.Results1.A total of 6,419 nurses from 23 general hospitals(three tertiary and 20 secondary)or 11 specialty hospitals(five tertiary and six secondary)returned the questionnaire with the validate rate of 95.9%(N=6158).The mean age of them was 30.56(standard deviation 7.014)years old,and 2.5%were male nurses.2.The most frequently reported missed care was food preparation(51.8%),warm food(39.9%),bathing(37.7%),ambulation(37.7%).turning(18.3%)and psychological support(18%),while the most rarely reported was drainage care(3.5%)and sample taking(3.2%).Staffing(73.8%),material resources(68.8%)and communication(67.5%)were main reasons for missed care.3.The analyses revealed following group differences with statistical significance(all P<0.05):a.total and five dimensional(i.e.basic nursing, nursing intervention, nursing plan, nursing assessment, hand washing)scores of missed nursing care between groups by age, marriage, number of children, years of nursing work, years of present ward work, professional rank, position, category of contract, average number of monthly night shifts;extra work during last three months, resign plan, family support, and friend support, respectively;and b.total and three dimensional(i.e.basic nursing, nursing intervention, hand washing)scores of missed nursing care,respectively.4.The binary logistic regression analysis showed that being male(OR=2.866),attending nurse(OR=4.128),having over 10 night shifts monthly(OR=2.610),resigning plan(OR=3.415),and little friend support(OR=2.110)were included in the model contributing to 98.6%of the variance of missed nursing care.5.A significant negative correlation(r =-0.334,P<0.001)was identified between the total score of missed nursing care and that of nursing job satisfaction. The highest negative correlation(r =-0.352,P<0.001)was found between the total score of missed nursing care and dimensional score of career opportunities(r =-0.352,P<0.001),while the lowest correlation(r =-0.087,P<0.001)was found between the dimensional score of hand washing and that of work-family balance.ConclusionMissed nursing care was existing in city level hospitals, especially missed basic nursing care.A significant negative correlation was detected between missed nursing care and nursing job satisfaction, while being male, attending nurse, over 10 night shifts monthly, resigning plan and little friend support were contributory factors to the risk of missed nursing care, suggesting the key areas for intervention in the future.
