Objective:Alzheimer's disease(AD)and Vascular Dementia(VaD)are the two most common types of dementia.It is thought that AD and VaD are two distinct dementias,but in recent years,more and more studies have shown that there are many similarities between these two types in terms of pathogenesis,risk factors and clinical features,which makes it more difficult to make differential diagnosis in some cases.This article aims to explore the correlation between relevant factors and concerning AD and VaD,and further seek clinically convenient indicators as identification marks,thus contributing to clinical work.Methods:We've selected 50 hospitalized patients diagnosed with AD in the second affiliated hospital of dalian medical university from September 2014 to November 2018,including 25 cases of male and 25 cases of female with the average age of(73.7±8.8).46 hospitalized patients in the same period diagnosed with VaD in the second affiliated hospital of dalian medical university have also been selected,including 23 cases of male and 23 cases of female with the average age of(70.2±9.7).Besides,we've also randomly selected 49 hospitalized patients in the same period with normal cognitive function in the second affiliated hospital of dalian medical university,including 25 case of male and 24 cases of female with the average age of(71.7±8.0).By drawing the morning fasting venous blood after admission,total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),apolipoprotein A I(ApoA I),apolipoprotein B(ApoB),blood glucose(GLU),hemoglobin A1C(HbA1C),creatinine(Cr),uric acid(UA),cystatin C(Cys C),and the ratio of ApoB/LDL-C were calculated.The differences of the above test results in the AD group,the VaD group and the normal cognitive function group were compared,and the correlation between the ratio of ApoB/LDL-C and MMSE score in the AD group and the VaD group was further analyzed.Results were statistically processed with SPSS25.0 software.Results:In terms of smoking,hypertension and diabetes,the rate of hypertension,diabetes and smoking was the highest in the VaD group,followed by the normal cognitive function group,and the lowest in the AD group.There was statistical difference between the VaD group and the normal cognitive function group and the AD group(P<0.017),while there was no statistical difference between the AD group and the normal cognitive function group(P>0.017).In terms of blood lipid,TC,LDL-C,HDL-C and ApoA I were the highest in the AD group,followed by the normal cognitive function group,and the lowest in the VaD group.There were statistical differences between the AD group and the normal cognitive function group and the VaD group(P<0.05),but no statistical differences between the VaD group and the normal cognitive function group(P>0.05).TG was the highest in the VaD group,followed by the normal cognitive function group,and the lowest in the AD group,but there was no statistical difference(P>0.05).ApoB was the highest in AD group,followed by VaD group,and the lowest in normal cognitive function group,but there was no statistical difference(P>0.05).ApoB/LDL-C was the highest in the VaD group,followed by the normal cognitive function group,and the lowest in the AD group.There was a statistical difference between the VaD group and the AD group(P<0.05),and there was no statistical difference between the two groups and the normal cognitive function group(P>0.05).In terms of blood glucose,GLU was the highest in the VaD group,followed by the normal cognitive function group and the lowest in the AD group,but there was no statistical difference(P>0.05).HbA1C was the highest in the group with normal cognitive function,followed by the VaD group and the lowest in the AD group,but there was no statistical difference(P>0.05).In terms of renal function,Cr and Cys C were the highest in the VaD group,followed by the AD group,and the lowest in the normal cognitive function group,but there was no statistical difference(P>0.05).UA was the highest in the VaD group,followed by the normal cognitive function group and the lowest in the AD group,but there was no statistical difference(P>0.05).In VaD group,the ratio of ApoB/LDL-C was correlated with MMSE score(R=0.462,P=0.001),while in AD group,there was no significant correlation between ApoB/LDL-C and MMSE score.The scatter plot and 95%confidence interval(R~2=0.214)of ApoB/LDL-C and MMSE score in VaD group were further plotted.Conclusion:Patients in the VaD group were more likely to be associated with smoking,hypertension and diabetes than those in the AD group and the cognitive function group,There were statistical differences between the AD group and the normal cognitive function group and the VaD group.while the TC,LDL-C,HDL-C and ApoA I in the AD group were all higher than those in the cognitive function group and the VaD group,There were statistical differences between the AD group and the normal cognitive function group and the VaD group.In the VaD group,the ratio of ApoB/LDL-C was significantly higher than that of the AD group and the cognitive function group,and there was a statistical difference between the VaD group and the AD group.In the VaD group,the ratio of ApoB/LDL-C was positively correlated with the MMSE score(R~2=0.214).Therefore,the ratio of ApoB/LDL-C may become a clinical indicator for the differentiation between AD and VaD,and a potential predictor of the degree of cognitive impairment in patients with VaD.
