Objective: Cholelith disease is common, frequent with clinical biliary system diseases, according to clinical symptoms of the disease, which recorded in ancient medical books, but about the prevention and treatment of this disease is still a big problem in today’s medicine, and treatment based on syndrome differentiation is the basic principles of traditional Chinese medicine to treat disease, with the development of modern medical technology, for the diagnosis of this disease is no longer limited to traditional inspection, auscultation and olfaction, inquiry, and pulse-taking and palpation, existing ultrasonic,X-ray,CT and MR,ERCP is more simple, rapid, accurate, mature.Based on the collection and observation of patients with cholelithiasis, this study preliminarily analyzed the internal relationship between the distribution pattern of TCM syndromes and ultrasound in patients with cholelithiasis, and mastered the change pattern of cholelithiasis,so as to provide a more solid research foundation for the TCM syndromes and clinical treatment of cholelithiasis. Methods:According to the research purpose and related diagnostic criteria of traditional Chinese and western medicine, patients cholelith disease clinical observe related data table, using the method of retrospective study, collected during October 2018 to December 2016 in XX hospital CGMH outpatient and the inpatient and physical examination center by B ultrasonic diagnosis of cholelith disease patients, patients’ general information,TCM four diagnosis information collection and imaging data of the 94 patients who met inclusion and exclusion criteria, use of Excel,SPSS22.0 analysis software for data sorting and statistical method,To analyze the clinical characteristics of patients with cholelithiasis and the relationship between TCM syndromes and imaging.Results:(1)Among the subjects of this study, there were no excluded or excluded cases,a total of 94 cases, among which the incidence rate of women was higher than that of men,and the overall age of onset was 56.88 17.85 years old,58.70 18.40 years old for women, and 54.82 17.17 years old for men.(2)Among the subjects of this study, the position of calculi was from high to low in the order of 61 cases of gallbladder calculi,22 cases of intrahepatic bile duct calculi,8 cases of gallbladder neck calculi, and 3 cases of common bile duct calculi. There was no significant gender difference.(3)In this study, the frequency of TCM syndromes was from high to low as follows: hepatobiliary dampness-heat syndrome > liver stagnation and qi stagnation syndrome > liver Yin deficiency syndrome > blood stasis arrest syndrome > thermal toxicity accumulation syndrome.(4)In this study, there were significant differences in TCM syndrome types, age and gender(P < 0.05),among which hepatobiliary damp-heat syndrome was more common in young men.Liver stagnation syndrome is more common in middle-aged women;Deficiency of liver-yin syndrome and blood stasis syndrome were more common in middle-aged and elderly people, and there was no significant gender difference. There was no significant age difference in thermal toxicity syndrome, which was mainly found in males.(5)In this study, the factors affecting cholelithiasis, including mood and stress, had statistical significance(P < 0.05),and the nature of work, tobacco, wine and tea preference, hypertension, diabetes, liver disease, body mass index and blood lipid were closely related to the occurrence of this disease.(6)In this study, there were differences between TCM syndromes and gallbladder size, gallbladder wall thickness and bile duct expansion(P < 0.05),but no significant differences between TCM syndromes and gallstone number and gallstone size(P > 0.05).Conclusion:1.The syndrome of dampness-heat of liver and gallbladder is more common in male patients with cholelithiasis, while the syndrome of stagnation of liver and qi is more common in female patients.2.The TCM syndrome differentiation of cholelithiasis is mainly characterized by dampness and heat of liver and gallbladder, stagnation of liver qi and qi,deficiency of liver Yin and blood stasis, and rarely heat toxicity.3.The TCM syndrome differentiation of cholelithiasis is closely related to the size of the gallbladder, the thickness of the gallbladder wall, and the dilation of the bile duct.Gallbladder size is more normal, gallbladder wall slightly thickened or thickened, part of the bile duct dilation; The gallbladder size was basically normal, the gallbladder wall was slightly thickened, and the bile duct dilation was not obvious. The gallbladder was narrowed, the gallbladder wall was thickened or slightly thickened, and the bile duct was not obviously dilated. The accumulation of heat toxicity showed obvious gallbladder enlargement, gallbladder wall thickening and bile duct dilation.
