Objective:To observe the clinical efficacy of unilateral and bilateral approach vertebroplasty(PVP)in the treatment of patients with III° osteoporotic vertebral compression fractures.Methods:A retrospective study of 136 patients with III° osteoporotic vertebral compression fracture(OVCF)treated by percutaneous vertebroplasty at the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from January 1,2017 to March 1,2018 Sexual analysis was divided into unilateral group(percutaneous unilateral pedicle puncture approach)and bilateral group(percutaneous bilateral pedicle puncture approach)according to the surgical approach.Observe the amount of bone cement injection,bone cement leakage,bone cement dispersion,postoperative pain improvement,vertebral height recovery rate and kyphosis improvement.The data obtained were analyzed by SPSS22.0 statistical software.Results: Among all the included subjects,47 were male and 89 were female;the age ranged from 60 to 91 years,with an average age of(75.3 ± 7.3)years.There were 64 cases in the unilateral group,including 21 males and 43females;aged 60-90 years,mean age(74.7±7.1);28 T6 vertebral bodies to T12 vertebral bodies in injured vertebrae,L1 There were 36 vertebral bodies to L5 vertebral bodies;72 cases in the bilateral group,including 26 males and46 females,aged 62-91 years,mean age(75.9±7.5);T7 vertebrae in injured vertebrae There are 31 body to T12 vertebral bodies,and 41 from L1 vertebral body to L5 vertebral body.There were no significant differences in the preoperative vertebral body height loss rate,vertebral height loss rate,vertebral kyphosis angle,preoperative VAS score and basic condition of the patients in both groups(P>0.05).Compared with the group,the unilateral group had shorter operation time,less X-ray irradiation and less bone cement injection,and the difference was statistically significant(P<0.05).Compared with preoperative,unilateral group and double The VAS scores of the lateral group were decreased at 3 days and at the last follow-up,and the difference was statistically significant(P<0.05).Compared with the unilateral group,the VAS scores of the patients in the bilateral group at 3 days and at the last follow-up were higher.Low,the difference was statistically significant(P<0.05);compared with preoperative,the vertebral height of the vertebral body,the loss rate of the middle height of the vertebral body,and the kyphosis angle of the vertebral body were small,the difference was statistically significant.Significance(P<0.05);the vertebral body height recovery rate,vertebral height recovery rate and kyphosis angle were smaller in the unilateral group than in the bilateral group(P<0.05);The diffuse volume fraction of bone cement in the lateral group was higher than that in the unilateral group,and the difference was statistically significant(P<0.05).Bilateral group,the cement leakage than multi-sided group,the difference was statistically significant(P <0.05).conclusion:It can be seen from the above that both the single and the bilateral approaches can quickly alleviate the pain of patients with III°OVCF,and have a certain reduction effect on the injured vertebrae;although the unilateral approach has shorter operation time and fewer X-ray radiation,Bone cement injection volume is less,bone cement leakage is relatively less,but the degree of bone cement dispersion,analgesic effect and injury of the injured vertebrae are better.Therefore,the author believes that III°OVCF patients are recommended to use percutaneous bilateral pedicle puncture vertebroplasty. |
