The Status And Influencing Factors Of Mental Health Among Nurses In A Tertiary General Hospital In Wuhan

Objectives(1)To understand the status quo of nurses' mental health in a general tertiary hospital.(2)To explore the correlation between nurses' mental health and social support.(3)To analyze the influencing factors of nurses' mental health,and to provide a reference for the nurse managers to develop targeted measures to improve the mental health status of nurses.MethodsFrom June 2018 to November 2018,a field questionnaire survey was conducted among 500 nurses from a tertiary grade A comprehensive hospital in Wuhan by using self-designed general questionnaire,symptom checklist 90(SCL-90)and social support rating scale(SSRS).SPSS 21.0 statistical software was used for data analysis,the measurement data were expressed as mean ± standard deviation,and the counting data were expressed as examples and percentages;t test and analysis of variance were used for comparison between groups;Pearson correlation analysis was conducted on the scores of nurses' mental health and social support;Multiple linear regression analysis was used to determine the influencing factors of nurses' mental health status.Results(1)A total of 500 questionnaires were distributed and 476 valid questionnaires were returned,the effective rate of the questionnaire was 95.2%.Actually,476 nurses were investigated,including 21 males,accounting for 4.41%,and 455 females,accounting for 95.59%.The average age of 476 nurses was(30.95 ± 7.02)years old,and the average nursing age was(7.61 ± 6.84)years.The education background was mainly bachelor degree or above,including 357 nurses,accounting for 75%;the work department was mainly intensive care unit,internal medicine system and surgical system,including 436 nurses,accounting for 91.6%;and the position was mainly clinical nurse,including 448 nurses,accounting for 94.12%.(2)The score of each factor of SCL-90 of 476 nurses were compared with the domestic norm,the results showed that the scores of somatization factor,compulsion factor,depression factor,anxiety factor,terror factor and mental illness factor of 476 nurses participating in this study were higher than the domestic norm(P<0.05),and the interpersonal relationship factor score was lower than the domestic norm(P<0.05).(3)The positive rate of different indicators of SCL-90 of 476 nurses: 136 nurses' total score of SCL-90 was greater than 160 scores,accounting for 28.57%;210 nurses' positive items were more than 43,accounting for 44.12%;the top three symptom factors with a factor score greater than or equal to two scores were compulsion,somatization and depression,with positive rates of 34.66%,26.26% and 22.06%,respectively.(4)Univariate analysis showed that there was a statistically significant difference in the scores of nurses' mental health status in terms of gender,educational background,department,family structure,occupational social status,labor compensation and professional knowledge and skill level(P<0.05).(5)The SSRS scores of 476 nurses were compared with the domestic norm,the results showed that the subjective support score,objective support score,support utilization score and total support score of 476 nurses participating in this study were lower than the domestic norm(P<0.05).(6)Univariate analysis showed that there was a statistically significant difference in the scores of nurses' social support status in terms of nursing age,professional tittle,marital status,occupational social status,labor compensation and professional knowledge and skill level(P<0.05).(7)Pearson correlation analysis showed that the scores of the dimensions of SSRS and the total score were negatively correlated with all the factor score of SCL-90,and the result was statistically significant(r=-0.846 ~-0.414,P<0.05).(8)The results of multiple linear regression analysis showed that educational background,occupational social status,professional knowledge and skill level and objective support were the influencing factors of nurses' mental health status(P<0.05).Conclusions(1)The scores of somatization,compulsion,depression,anxiety,terror and mental illness factors were higher than the domestic norm,and the positive rate of SCL-90 statistical indicators was higher than 14%,which indicated that the mental health status of nurses was in bad condition.(2)The scores of the dimensions of SSRS and the total score were lower than the domestic norm,which indicated that the social support of nurses needed to be enhanced.(3)The scores of the dimensions of SSRS and the total score were negatively correlated with all the factor score of SCL-90,namely nurses with high level of social support often had high degree of mental health,and vice versa.(4)The mental health status of nurses is influenced by multiple influencing factors,the nurse managers should actively formulate corresponding strategies based on controllable factors,and thereby promote their mental health and the quality of care work.
