| Objective:To investigate the value of CT and MRI in the differential diagnosis of benign and malignant thyroid nodules and its correlation with serum lipids and adiponectin levels.Methods:210 patients with thyroid nodules,who were referred to our hospital from April 2013 to April 2017,with complete clinical data,were selected as the research subject.In this cohort,a total of 240 lesions were all the thyroid nodules as confirmed by postoperative pathological biopsy.Meanwhile,all patients underwent preoperative conventional CT and MRI and were defined as CT group and MRI group.The results of CT group and MRI group were compared with the results of surgical pathology.In the same period,210 healthy cases were selected as the control group.We took 3mL of venous blood from the observation group on an empty stomach the next morning after admission,and we took 3 ml venous blood from the control group on an empty stomach in the same day,Then we measured the levels of serum adiponectin(APN),endolipids and TN F-a(TNF-a)by enzyme-linked immunosorbent assay after serum separation.Then we analyzed the correlation between APN,endolipids,TNF-a and the differential properties of thyroid nodules by CT and MRI.Results:All the 210 cases of thyroid nodules included were finally diagnosed.We detected 240 nodules:176 benign nodules(73.33%)and 64 malignant nodules.The topic twobenign thyroid diseases were hashimoto’s thyroiditis(30.11%)and nodular goiter(27.84%).The topic two malignant disease were papilla ry carcinoma(48.44%)and spindle cell tumor(31.25%).The coincidence rate of CT combined with MRI in the diagnosis of benign and malignant thyroid nodules was 99.58%(P>0.05),but slightly higher than that of single CT or MRI(P<0.05).The sensitivity of the diagnosis of thyroid nodule by CT was lower than that by MRI(P<0.05).The specificity of CT was higher than that of MRI(P<0.05).There was no significant difference in TNF level between the observation group and the control group(P>0.05).The APN level of malignant nodules was lower than that of benign nodules in the observation group(P<0.05).And the lipid level was higher in malignant nodules than that in benign nodules(P<0.05).The results of SPSS Pearson correlation analysis showed that the diagnosis of benign and malignant thyroid nodules by CT and MRI had no correlation with TNF-a(P>0.05),and negativecorrelated with APN(P<0.05),and positive correlated with endolipids(P<0.05).Conclusion:The evil nature of the differential diagnosis of thyroid nodules good application of CT and MRI diagnosis can obtain better diagnostic value,high diagnostic accuracy,and each has advantages,We could further improve the diagnostic coincidence rate if we combined them in clinical diagnosis.There had a certain correlation between CT and MRI when they were used to the differential diagnosis of benign and malignant thyroid nodules with APN and endolipids.If we enhanced the determination of APN and endolipid level we could remedy the disadvantages of CT and MRI diagnosis,and we could help patients to make early diagnosis,and provide basis and reference for clinical treatment of patients. |
