Study On The Correlation Between The Expression Of E3 Ubiquitin Ligase Hrd1 In The Tissues Of Triple-Negative Breast Cancer And The Clinico-pathological Characteristics And Prognosis Of Patients

Objective:HMG-CoA reductase degradation protein 1(Hrdl)is an E3 ubiquitin ligase.Studies have shown that Hrdl is indeed expressed in breast cancer tissues and may be closely related to the occurrence and development of breast cancer.This study attempted to analyze and compare the clinico-pathological characteristics and prognosis of Triple-Negative Breast Cancer(TNBC)patients with different Hrdl expression status,so as to speculate the significance of Hrdl in TNBC treatment prognosis analysis and other aspectsMethod:The specimens were from 126 TNBC patients admitted to the first affiliated hospital of Dalian Medical university from January 1,2002 to March 31,2010.There were 126 cases of carcinoma and paracancerous tissue.All patients underwent radical mastectomy for the first time and received no preoperative chemotherapy,radiotherapy or targeted therapy.The expression level of Hrdl was detected by immunohistochemical SP method.The pathological data included age of onset,histopathological type,histological grade,tumor size,lymph node status,TNM stage,etc.The survival status of the patients was determined by telephone follow-up and household registration examination.SPSS 20.0 software was used for statistical analysis of the data.The comparison among the categorical variables was conducted by chi-square test,and Fisher’s exact test was used if the chi-square test condition was not satisfied.Kaplan-meier method was used for the survival curve,and log-rank test was used to compare the difference of survival curves.Univariate and multivariate analysis was performed using COX risk scale model,and the test level was=0.05.Results:All the 126 breast cancer patients enrolled were female,including 67 cases of Hrd1+(53.2%)and 59 cases of Hrdl-(46.8%).Age was range 20-84 years old.Median age was 52 years old.33 cases were younger than or equal to 45 years old(26.2%).78 cases were older than 45 years old and younger than or equal to 65 years old(61.9%).15 cases were older than 65 years old(11.9%).Postoperative histopathological classification:there were 115 cases(91.3%)of invasive ductal carcinoma.2 cases(1.6%)were intraductal carcinoma.9 cases(7.1%)were other types.Tumor size:Tis were 2 cases(1.6%),T1 were 47 cases(37.3%),T2 were 65 cases(51.6%),T3 were 8 cases(6.3%),T4 were 4 cases(3.2%).Lymph node metastasis:there were 88 cases(69.8%)of N0,21 cases(16.7%)of N2,13 cases(10.3%)of N3,and 4 cases(3.2%)of N3.Clinical stages:stage 0 were 2 cases(1.6%)stage Ⅰ were 38 cases(30.2%).stage Ⅱ were 60 cases(47.6%).stageⅢwere 22 cases(17.4%).Stage Ⅳ were 4 cases(3.2%).Among the carcinoma tissues,67 cases(42.6%)were positive and 59 cases(57.4%)were negative.There were 26 cases(15.8%)with positive Hrd1 and 106 cases(84.2%)with negative Hrd1 in para-carcinoma normal breast tissue.In addition,Hrdl expression was correlated with tumor histological type(P<0.05),and had no significant correlation with histological grading of tumor size and clinical staging of lymph node metastasis and other clinico-pathological indicators(P>0.05).73 patients survived,32 died,21 lost follow-up,the follow-up rate was 83.3%.the lost follow-up rate was 16.7%,and the overall survival rate was 74.6%.The survival rate of Hrd1+patients was 74.6%,and that of Hrdl-patients was 74.6%.The comparison of the survival curves between the two groups showed that the difference in Hrd1 expression did not affect the OS of TNBC(P>0.05)Conclusion:1.The expression level of Hrdl in TNBC tissues was significantly higher than that in para-carcinoma tissues2.The histological types of TNBC patients with Hrd1+were mostly non-specific invasive carcinoma(invasive ductal carcinoma),while the special type of invasive carcinoma was mostly Hrdl-3.In TNBC patients,Hrdl expression was not significantly correlated with the age of onset,histological grade,tumor size,clinical staging,lymph node metastasis and other clinico-pathological indicators4.There was no statistical difference in OS between patients with Hrd1+and those with Hrd1-.
