| ObjectiveThis study focuses on the effects of varicocele on semen parameters and malondialdehyde(MDA)content in seminal plasma in patients.The methylenetetrahydrofolate reductase gene C667 T is in the population.The effect of polymorphism on sperm survival rate and MDA in the seminal plasma,and the polymorphism distribution of the methylenetetrahydrofolate reductase gene C667 T in the population has an effect on the population with VC.MethodsA total of 63 VC patients(left side)who were admitted to our hospital from February2017 to December 2018 were enrolled.According to the clinical classification,21 patients were in the I degree group,22 in the II degree group,and 20 in the III degree group.The control group was 20 patients with long prepuce without VC.The percentage of forward sperm,the content of MDA in seminal plasma and the content of reproductive hormone were compared in each group.Except the control group,the spermatic vein ligation was performed under microscope.The oxygen saturation of the venous blood of the first vein was ligated and the venous oxygen saturation of the last vein of the ligation was compared to the venous oxygen saturation between the two.According to the MTHFRC677 T genotype,the above 83 patients were divided into three groups: CC,CT and TT genotypes,31 cases of CC type,28 cases of CT type and24 cases of TT type.The sperm survival rate and MDA content in seminal plasma were compared in each group.The patients in the three genomes were subdivided according to the degree of VC,and were divided into the I degree varicose group,the II degree varicose group,the III degree varicose group,and the normal group.In each varicosedegree group,whether the three genomes had statistics were compared.Finally,patients with a spermatic vein diameter greater than 2.5 mm(53 patients)were divided into mildly weak spermatozoa and severely weak spermatozoa according to the degree of weak spermatozoa,and mild weak spermatozoa referred to a grade b spermatozoa<40%.,but 20%,of which a grade sperm <25%,but > 5%;severe weak sperm group refers to grade a b sperm <20%,of which a grade sperm <5%,and then divided into three groups according to genotype.ResultsThe percentage of forward sperm motility in each group was(40.1±16.9%),in the I degree group(38.9±17.4%),in the II degree group(29.6±14.1%),and in the III degree group(24.3±12.2%).Compared with the control group,there was no significant decrease in the I degree group,no statistical difference(P>0.05),and the percentage of the forward sperm activity in the II degree and III degree groups decreased significantly,which was statistically significant(P< 0.05),and the difference between the two groups was statistically significant(P<0.05).The MDA content in each group of seminal plasma was control group(1.05±0.060nmol/mL),I degree(1.41±0.078nmol/mL)and II degree(1.83±0.049 nmol/mL),and III degree(2.77).±0.052 nmol/mL),compared with the control group,the MDA content of the seminal plasma of the I degree,II degree and III degree group were increased,which was statistically significant(P<0.05).The difference between the groups was statistically significant(P<0.05).The reproductive hormones of each group were determined separately;The content of Serum prolactin(PRL)was in the control group(227.31±12.71μIU/ml)and I degree group(226.48±34.53).μIU/ml),II degree group(231.32±39.12μIU/ml),III degree group(225.35±36.44μIU/ml),and the content of estradiol(E2)in each group were control group(38.8± 4.31 pg/ml),I degree group(43.2±12.78 pg/ml),II degree group(46.91±15.72 pg/ml),III degree group(48.40±14.17 pg/ml).There was no significant difference in serum PRL and E2 levels between the groups(P>0.05).The content of follicle stimulating hormone(FSH)in each group was control group(3.93±0.86 mlU)./ml),I degree group(4.32±0.94 mlU/ml),II degree group(6.98±0.72 mlU/ml),IIIdegree group(7.32±1.05 mlU/ml),the content of luteinizing hormone(LH)in each group was control group(4.86±0.45 mlU/ml),I degree group(5.32±1.29 mlU/ml),II degree group(8.45±1.43 mlU/ml),and III degree group(9.24±1.82 mlU/ml).Compared with the control group,the levels of serum FSH and LH were not significantly increased in the I degree group,and there was no significant difference(P>0.05),follicle stimulating hormone(FSH)and luteinizing hormone in the II degree group and the III degree group.LH)was elevated,which was statistically significant(P<0.05).There was no significant difference between the two groups(P>0.05).The content of testosterone(T)in each group was control group(4.45±0.42 ng/ml),I degree group(4.02±0.73ng/ml),II degree group(3.17±0.54 ng/ml),III degree group(2.63±0.96 ng/ml),compared with the control group,The T content of the I,II and III degrees increased which was statistically significant(P<0.05),the difference between the groups was statistically significant(P<0.05);The first venous oxygen saturation of the I degree group,the second degree group and the third degree group were measured respectively.And the last venous oxygen saturation,the value of the I degree group was(40.1 ±12.4%),(90.4 ± 2.8%);the value of the II degree group was(35.3 ± 5.8%),(88.7 ± 4.6%)The values ??of the III degree group were(27.7±10.1%)and(86.8±8.7%),respectively.Before and after surgery,the blood oxygen saturation of venous blood increased significantly,which was statistically significant(P<0.05).There was a significant difference between the first venous oxygen saturation in the degree group,the second degree group and the third degree group,which was statistically significant(P<0.05).The content of MDA in each genomic seminal plasma was CC genome(1.21±0.908nmol/mL),CT genome(1.43±0.954 nmol/mL),TT genome(1.67±1.293nmol/mL),andthe difference of MDA content between the groups was statistically significant(P<0.05).Among the three genomes of CC,CT and TT,the sperm motility of each genome was(89.8±2.7%)in the CC genome and(82.1±3.1 in the CT genome).%),TT genome was(73.3±2.9%),the difference of sperm survival rate in each group was statistically different(P<0.05).Finally,there were no significant differences between the three groups of CC,CT and TT in the I degree varicose group,the II degree varicose group,the III degree varicose group and the normal group(P>0.05).The three genomes of CC,CT and TT were statistically different(P<0.05).Compared with CC genotype,the sperm genotype of CT genotype decreased,while the TT genotype Further significantly lower.ConclusionOxygen free radicals,insufficient oxygen supply,and decreased serum T content may be the mechanism by which VC affects testicular spermatogenesis;MTHFR gene polymorphism is associated with sperm survival rate and oxygen free radical levels in seminal plasma.Oxygen free radicals may be one of the common mechanisms of VC and MTHFR gene polymorphisms affecting testicular function.There is no correlation between MTHFRC677 T polymorphism and VC.MTHFRC677 T polymorphism is likely to be a susceptibility factor for decreased semen quality. |
