BackgroundStroke is common disease of neurology.Post-stroke cognitive dysfunction is important for stroke rehabilitation.Post-stroke cognitive dysfunction is related to many factors,gender,age,heredity,atrial fibrillation,stroke history,hypertension,diabetes,TIA,etc.are associated with post-stroke cognitive function,but whether white matter lesions are associated with post-stroke cognitive dysfunction,there is currently insufficient evidence to suggest that However,there is insufficient study about relation between white matter lesions and post-stroke cognitive dysfunction.ObjectiveWe used meta-analysis to assess the association between white matter lesions and post-stroke cognitive dysfunction,and explored the risk factors associated with post-stroke cognitive dysfunction as well.MethodsWe searched literature in Pubmed,Medicine,China Knowledge Network,Wanfang Database and Weipu Database from the establishment of the database to April 2018.Meta-analysis was performed using RevMan 5.3 software after screening the literature,extracting data,and evaluating the quality of the included studies according to the inclusion and exclusion criteria,to explore the correlation between post-stroke cognitive dysfunction and white matter lesions.ResultsA total of 469 related literatures were detected,of which 416 were excluded for the fact that stroke,cognitive impairment,and white matter lesions were not studied simultaneously.37 cases were excluded from the research due to non-clinical trials and no relation was found between stroke and white matter lesions.10 articles were excluded owing to the incomplete data and non-data.Eventually,6 articles were included in the study.The quality of the literature was evaluated and the overall quality of the literature was moderate.Meta-analysis showed that the degree of white matter lesions was negatively weakly correlated with cognitive function after stroke(r =-0.318 95% CI=(-0.40,-0.25)).The degree of white matter lesions and memory function was negatively weakly correlated(r =-0.318 95%CI=(-0.36,-0.17)).The degree of white matter lesions was negatively correlated with executive function and attention(r =-0.310 95%CI=(-0.43,-0.22),r =-0.345 95%CI=(-0.59,-0.13)),and the correlation was weak as well.ConclusionThis research showed that,patients with stroke no matter in the acute phase or the recovery phase,the more severe in the white matter lesions,the more obvious the decline of cognitive function will be,especially in memory,execution,and attention.There are certain relationships between white matter lesions and dimension about visual space,language,orientation,abstraction,and naming,but there is no conclusion yet. |
