Application Of The Flexible Lightwand Combined Intubating Laryngeal Mask Airway In Intubation In Elderly Patients With Mallampati Score 3 To 4

Objective:To explore the feasibility and outcome of flexible lightwand combined intubating laryngeal mask airway(ILMA)in intubation in elderly patients with Mallampati score 3 to 4.Methods:Sixty patients(ASA I-III,60-83 years,and Mallampati score 3-4),undergoing elective surgeries were enrolled in this study.All patients were randomly selected into two groups:flexible lightwand intubation group(Experiment group,n=30)and traditional lightwand guided intubation group(Control group,n=30).Hemodynamic parameters including systolic blood pressure(SBP),diastolic blood pressure(DBP),and heart rate(HR),were recorded at the following time points:before,immediately after anesthesia induction,1 min,3 min,and 5 min after incubation.Intubation time,success rate of the first time incubation,overall success rate of intubation,and complications such as sore throat and hoarseness were also recorded.Results:There were no significant difference in terms of patients’ gender,age,weight,etc,(P>0.05).Compared with patients ’ the parameters before anesthesia induction,the SBP,DBP,and HR of immediately after anesthesia induction,1 min,3 min,and 5 min after intubation in two groups showed no significant changes(p>0.05).All patients with tracheal intubation were completed successfully.There was no significant difference between control and experiment group in the success rate of first time incubation.Compared to the control group,experiment group had longer average operation time and lower incident rates in sore throat.Conclusion:Flexible lightwand combinedILMA in intubation had a longer operation time but fewer complications for elderly patients with Mallampati score 3 to 4.
