| Objective This research investigates the differences between the indicators of frequent and infrequent exacerbation of chronic obstructive pulmonary disease(COPD),such as blood routine、results of sputum culture and arterial pressure of carbon dioxide inartery(PaCO2),analysing the relationship in COPD to predict the risk of acute exacerbation and guide the treatments individually.Methods By using retrospective analysis,the research selected 102 patients(66 males and 36 females,aged from 50 to 90)with COPD who diagnosed in respiratory medicine department(outpatient and inpatient department)of No.2 hospital of Anhui Province during February 2017 to October 2017,according to the diagnostic criteria ofGuidelines for the Diagnosis and Treatment of Chronic Obstructive Pulmonary Disease(revised in 2013)which is developed by Chinese Medical Association.On the basis of the frequency of acute exacerbation in the previous year,102 patients were divided into two groups,frequent exacerbation group(≥2 times/year,55 patients)and infrequent exacerbation group(<2 times/year,47 patients).Venous blood was extracted from all the patients before treatment to test blood routine.Arterial blood was extracted to test arterial blood gas analysis.The sputum of all patients before treatment was retained for bacterial and fungal culture.SPSS20.0 statistical software was used for statistical analysis of the test results.By virtue of independent sample t test,χ2 test,and multiple Logistic regression analysis,it compared the differences between indicators of frequent and infrequent exacerbation of COPD,to explore the the relation of acute exacerbation,for example white blood cell count(WBC)、neutrophil count(NEUT)、lymphocyte count(LY)、neutrophil percentage(NEU%)、lymphocyte percentage(LY%)、eosinophils count(EOS)、basophils count(BASO)、eosinophils percentage(EOS%)、basophils percentage(BA%)、Neutrophil-to-Lymphocyte ratio(NLR)、Eosinophil-to-Basophil ratio(EBR)、platelet count(PLT)、mean platelet volume(MPV)、PaCO2 and sputum culture.Results1.NEUT,NEU%and NLR of frequent exacerbation group were higher than those of infrequent exacerbation group,while LY and LY%were lower(P<0.05).2.NLR and PaCO2 were risk factors for frequent exacerbation of COPD.Increase of NLR and PaCO2 lead to an increasing risk of frequent exacerbation of COPD(P<0.05).3.The positive rate sputum microorganism culture(bacteria+fungi)of frequent exacerbation group is higher than that of infrequent exacerbation group(P<0.05).Conclusion1.The levels of NLR and PaCO2 in COPD patients with frequent exacerbation are higher than those in patients with infrequent exacerbation.As a consequent,NLR and PaCO2 could be considered risk factors for frequent exacerbation of COPD.2.Such patients with frequent exacerbation may be vulnerable to candida albicans distributions. |
