Objective: To observe the clinical outcomes of self-made external fumigated-washing therapy,named Osteomyelitis Ⅱ Formula for External Wash,in combination with focus debridement and vancomycin-calcium sulfate implantation in the treatment of chronic calcaneal osteomyelitis induced by methicillin-resistant Staphylococcus aureus(MRSA),and to compare the clinical efficacy in treatment group and control group,and to observe clinical efficacy to seek targeted therapies for osteomyelitis infected by drug-resistant bacteria.Methods: A total of 60 patients with chronic calcaneal osteomyelitis induced by MRSA from January 2016 to October 2017 were selected in the treatment center for osteomyelitis in Luoyang Bonesetting Hospital(Henan Orthopedic Hospital),including 33 men and 27 women,with age range from 18 to 55 years old and an average age of 40.6 years old,disease duration ranged from 3 months to 2 years and 10.9 months for average.And all these patients met inclusion criteria,complicating with no other primary diseases,were randomly divided into treatment group and the controls,with 30 cases per group.The treatment group was continuously given fumigated-washing therapy per day in affected area after admission until 1 d before operation,while the control group was only given daily routine dressing changes,and the therapies of two groups were lasted for one week.Afterwards,patients of two groups underwent focus debridement and vancomycin-calcium sulfate implantation,routine wound dressing changes for every other day,and follow-up after discharge.Postoperative indicators of two groups were observed and recorded,and gender,age,duration of disease,bacterial culture results,postoperative results of laboratory tests,total response rate,recurrence rate,and clinical symptom scores of traditional Chinese medicine(TCM)were statistically analyzed,and clinical efficacy of two groups were compared.Results: There was no significant difference in gender,age,and disease duration between the two groups(P>0.05);bacterial culture results,postoperative results of postoperative indicators,the total response rate of treatment,the recurrence rate,and symptom scores of TCM showed significant between-group differences,and the treatment group was superior to the control group(P<0.05).Conclusions: External fumigated-washing therapy,named Osteomyelitis Ⅱ Formula for External Wash,in combination with focus debridement and vancomycin-calcium sulfate implantation has better clinical efficacy than western medicine alone in the treatment of calcaneal osteomyelitis induced by MRSA(syndrome differentiation as healthy-qi deficiency and evil-qi stagnation).External fumigated-washing therapy can effectively improve local symptoms,control local inflammatory response,inhibit resistant bacteria,effectively enhance curative rate and decrease the relapse of chronic osteomyelitis induced by MRSA.
