Objective:By comparing the use of platelet-rich fibrin(PRF)and the natural healing process of blood clots in extraction socket,the differences in the early healing and regeneration of gingival soft tissue of the extraction wound were observed,and the effects of PRF in reducing the early inflammatory response and its ability in inducing the regeneration of soft tissue were evaluated.Methods:Patients undergoing orthodontic treatment and in need of extracting the maxillary or mandibular first premolars from the Stomatology Department of the First Hospital of Shanxi Medical University were selected.They consisted of 8 males and 12 females,aged between 12 to 25 years old,with an average age of 18.5.The premolars of the same patient on the same jaw were divided into 2 groups,i.e.experiment group(PRF gel + PRF membrane)and control group(simple blood clot),and the corresponding extraction wounds were treated respectively.Twenty milliliters of blood from the patient’s median cubital vein was taken 10 minutes before tooth extraction to obtain PRF.Teeth of the same position on the opposing sides of the same jaw were extracted with local anesthesia and minimal invasion and the extraction sockets were scratched.Extraction sockets of the experimental group were filled with PRF gel and PRF membrane,while the those of the control group were filled with blood clots.The patients were followed up for 3,7 and 14 days after surgery to observe the soft tissue healing area of the bilateral alveolar gingival.observation of buccal palsy paralysis height in alveolar sulcus after 3 days and 14 days after operation.Photographs of the buccal and maxillofacial regions of the bilateral alveolar socket using a ring-flash SLR camera.The Photoshop CS6 software was used to measure the alveolar fossa healing area and buccal keratinization height.The mucosal status(color and degree of swelling)of bilateral tooth extraction was recorded on the 3rd and 7th days after operation.Results:1.In 20 patients,40 first premolars were successfully removed.There was no root crushing and no tearing of the gums during the operation.During the observ ation period,the soft tissue of the extraction socket healed well,no PRF sheddi-n g and dry socket symptoms,and good patient compliance.The oral imaging data could be retained on time2.The color and swelling degree of the mucosa in the PRF group were better than those in the normal blood clot group on the 3rd day after operation(P<0.05).The color of mucosa in the PRF group was better than that in the normal blood clot group at 7 days after operation(P<0.05),but there was no significant difference in the degree of mucosal swelling(P>0.05).3.There was a significant difference in the healing area between the PRF group and the normal blood clot group at three different time points(P<0.05).Conclusion:1.PRF can reduce the early inflammatory reaction of the gingival soft tissue after extraction.2.PRF can prevent keratosis retraction early after tooth extraction3.PRF can accelerate the early healing of the soft tissue of the alveolar gingiva after extraction.
