| Objective :By analyzing various factors that may cause local recurrence of rectal cancer after radical resection of rectal cancer,we discussed the risk factors of local recurrence of rectal cancer after radical resection.We also studied the survival benefits of reoperation in patients with local recrudescence after rectal cancer surgery.Methods :The clinicopathological and survival data of 82 patients who received radical resection of rectal cancer from August 2008 to August 2013 in a group of gastrointestinal cancer surgery in the affiliated Cancer Hospital of Fujian Medical University were analyzed retrospectively.The related factors of local recurrence were analyzed by univariate and multivariate analysis.Life-Table method was used to calculate cumulative survival rate and Kaplan-Meire method was used to describe survival curve.Log-ranK test was used to compare the survival differences between the two groups.Results:Univariate analysis showed that preoperative CEA level,tumor location,differentiation degree and lymph node metastasis were related factors to local recurrence of rectal cancer(P < 0.05).Multivariate logistic regression analysis showed age,preoperative CEA level and differentiation degree.Lymph node metastasis was an independent risk factor for local recurrence of rectal cancer after operation(P < 0.05).The recurrence rate was 37.8in the local recurrence group(31 cases).The recurrence time was 1-56 months,and the mean recurrence time was 15.2 months.24 cases(77.4%)recurred within 2 years.The 5-year survival rate was 33.33% and 12.5% respectively.The difference was statistically significant(P < 0.05).Conclusion:Preoperative CEA level,tumor location,differentiation and lymph node metastasis were the risk factors for local recurrence of rectal cancer.Active re-operation,especially R0 resection,can benefit the survival of recrudescent rectal cancer patients. |
