| Objective:Based on the perspective of the medical side,we understand the current status and existing problems of the Job stability of physicians in the community health service centers under the background of vigorously promoting the supply-side reforms in the medical and health field,and then discuss the path of the influence of physicians’ Job stability,Satisfaction and professional identity and build a relationship model between the three to explore the relevant influencing factors of the mechanism,so as to provide a targeted strategy to stabilize the community health center physicians,promote the rational distribution of grassroots health personnel,Change the dilemma of shortage of health personnel and frequent mobility at the grassroots.Methods:This research combines empirical research,interviews and other research methods.On the one hand,it collected the flow of physicians from community health service centers in Fuzhou City from 2014 to 2016,and on the other hand,it investigated the current situation of the job stability,Satisfaction and professional identity among 330 physicians in the community health service center.Statistical description and statistical inference were used to analyze the results of the questionnaire survey,and a structural equation model of the job stability,Satisfaction and professional identity was constructed.Through interviews,the relevant people’s views and opinions on the related contents of the work of the physicians in the community health service centers were understood.Results:(1)In 2014-2016,A total of 215 physicians flowed in and 119 physicians flowed out in the community health service center in Fuzhou City.The mobile physicians were mainly young and middle-aged,junior title,and undergraduate students.(2)The overall job satisfaction score of community physicians in Fuzhou City was 3.58,with the highest interpersonal relationship(3.71)and the lowest wage and welfare(2.90).There was a statistically significant difference in overall satisfaction and satisfaction scores in each dimension among physicians of different genders,ages,places of origin,highest academic qualifications,average monthly income,job title,nature of the job,length of service of physicians.Multiple linear regression analysis showed that there was a statistically significant difference in overall satisfaction scores among physicians in terms of job nature,average monthly income,and highest education level.(3)The overall professional identity score of community physicians in Fuzhou City was 3.62.Univariate analysis showed that there was a statistically significant difference in the overall professional identity scores among physicians of different ages,places of origin,highest academic qualifications,average monthly income,and nature of the job.The results of multiple linear regression analysis showed that the scores of physicians’ overall professional identity scores were statistically significant among the highest academic qualifications,average monthly income,and nature of the job.(4)The overall stability score of community physicians in Fuzhou City was 3.30.Univariate analysis showed that there was a statistically significant difference in overall stability scores among physicians of different ages,places of origin,highest academic degrees,and average monthly income.The results of the mixed-linear model show that wages and benefits,interpersonal relationships,leadership and management dimensions,and overall professional identity in satisfaction are significant predictors of job stability.(5)Structural equation model results show that after eliminating some of the invalid paths,job satisfaction and professional identity have a direct positive effect on job stability,and professional identity plays an indirect effect in the path of job satisfaction affecting job stability.Conclusion: At present,physicians in Fuzhou community health service center have the status of normal Job stability,Job satisfaction and professional identity level are medium,and the influencing factors of the three are not the same.There is a close correlation of the job stability,Satisfaction and professional identity among physicians,and there is a certain path relationship.we should adopt targeted intervention measures to stabilize the community physician team and improve the level of primary health care services from the perspectives of various stakeholders including government departments,all sectors of society,institutions,and physicians. |
