ObjectiveBy comparing the levels of HPV16 E7 protein in normal cervix,low grade and high grade cervical intraepithelial lesions and cervical cancer,the relationship of HPV16 E7 protein concentration in different degrees of cervical lesions was evaluated statistically to explore its clinical application value in screening cervical lesions.It provides a reference for the early detection,early diagnosis,early treatment,and evaluation of the disease and prognosis of cervical cancer.MethodsThe first department,we selected 100 patients with HPV16-positive by HPV DNA test from July 2016 to December 2016 in the Tianjin Central Hospital of Gynecology and Obstetrics as the first group,aged 20-72 years,with an average age of 42.76 years.The cervical exfoliated cells were taken and HPV16 E7 protein was detected by enzymatic chemiluminescence.All patients underwent colposcopy and cervical biopsy was performed.Using pathological diagnosis as the gold standard,the HPV16 E7 protein concentration in different degrees of cervical lesions was compared.The accuracy of HPV16 E7 protein detection method used in this study was verified by statistical methods,and the cut-off value of HPV16 E7 protein concentration in different degrees of cervical lesions was calculated.The second department,we select 100 patients with HPV16-positive HPV DNA from December 2016 to March 2018 in the Tianjin Central Hospital of Gynecology and Obstetrics,aged 22-67 years,with an average age of 41.88 as the second group.The same method was used to detect HPV16 E7 protein and colposcopy and cervical biopsies were taken.This group of patients were divided into two groups according to the cut-off value,and the pathological diagnosis was the gold standard.The value of HPV16 E7 protein in the detection of high-grade cervical intraepithelial lesions and cervical cancer was compared with the efficacy of TCT screening for cervical lesions.,Discuss the clinical application value of HPV16 E7 protein detection in cervical lesion screening.Results1.In the first department of 100 patients included in this study,the correlation analysis of HPV16 E7 protein expression levels in different degrees of cervical intraepithelial lesions and cervical cancer results showed that the expression level of HPV16 E7 protein was positively correlated with the extent of cervical lesions.The correlation was significant(r=0.589,P<0.01).2.For the first department of patients in this study,the HPV16 E7 protein concentration was significantly higher in the high-grade cervical intraepithelial lesions and cervical cancer group than in the normal and low-grade cervical epithelial lesions of the cervix.There was statistical difference between the two groups(P<0.01).3.In the first department of 100 patients in this study,the accuracy of the HPV16 E7 protein test in cervical high-grade intraepithelial lesions and cervical cancer was evaluated using an accuracy test(ROC curve).The area under the ROC curve was AUC = 0.817 with a confidence interval of(0.729,0.904),between 0.70-0.9.The results showed that according to the sample size and grouping status in this study,the detection of HPV16 E7 protein had better accuracy in detect the normal,low-grade cervical intraepithelial lesions and high-grade cervical intraepithelial lesions,cervical cancer.After calculating the Youden index as HPV16 E7 protein concentration = 8.68 ng/ml,that is: HPV16 E7 protein concentration of 8.68 ng/ml can be used as a critical value to distinguish high grade cervical intraepithelial lesions and cervical cancer.4.The second department of 100 patients included in this study were divided into two groups according to the cut-off value.The results showed that the sensitivity,specificity,positive predictive value,and negative predictive value of HPV16 E7 protein in the diagnosis of cervical high-grade intraepithelial lesion or cervical cancer were 87.14%,70.00%,87.14%,and 70.00%,respectively.5.The second department of 100 patients in this study were divided into TCT results.The results showed that the sensitivity,specificity,positive predictive value,and negative predictive value of TCT in the diagnosis of cervical high-grade intraepithelial neoplasia/cervical cancer were: 75.71%,63.33%,82.81%,52.78%.6.HPV16 E7 protein and TCT were compared with the pathological examination,HPV16 E7 protein showed higher consistency with pathological findings than TCT(0.571 and 0.369,respectively).Conclusions1.The expression of HPV16 E7 protein was positively correlated with the degree of cervical lesions.As the extent of cervical lesions increased,the expression level of HPV16 E7 protein also increased.2.The HPV16 E7 protein concentration was significantly higher in the high-grade cervical intraepithelial neoplasia and cervical cancer group than in the normal cervicaland low-grade cervical intraepithelial neoplasia groups.3.The enzymatic chemiluminescence method used in this study to detect HPV16 E7 protein expression in cervical exfoliated cells has a good accuracy.4.HPV16 E7 protein detection has better sensitivity,specificity,positive predictive value and negative predictive in the diagnosis of cervical squamous intraepithelial lesions and cervical cancer.5.HPV16 E7 protein detection has higher diagnostic value than TCT detection..
