ObjectiveBy analyzing the variation of the electroencephalogram(EEG)coherence of the chronic insomnic patients with and without anxiety and depression in rest and cognitive task status,to study the characteristics of the cortical activating synchronization and the features of cerebric functional connection in the two group and to explore the electrophysiological indexes for the clinical diagnosis and treatment of insomnia and cognitive dysfunction.MethodFifty-three chronic insomnic patients between 40-65 years old which met the ICSD-3 diagnostic criteria were selected,including twenty-eight chronic insomnic patients with anxiety and depression,twenty-five chronic insomnic patients without anxiety and depression and twenty volunteers were recruited as the healthy control.The insomnia severity index(ISI),pittsburg sleep quality index(PSQI),epworth sleep scale(ESS),hamilton anxiety scale(HAMA),hamilton anxiety scale(HAMD),mini-mental State Examination state examination(MMSE),montreal cognitive assessment(MoCA),trial making test(TMT),color word test(CWT)and verbal learning test(AVLT)were used to assess all the subjects.The EEG examination was performed in the rest and cognitive task status of all subjects and the EEG signals were divided into 7 frequency bands as the frequency from low to high,namely δ(0-3.5Hz)θ(3.5-7.5Hz)α1(7.5-10.5Hz)α2(10.5-13.5Hz)β1(13.5-18.5Hz)β2(18.5-30.5Hz)γ(30.5-79.5Hz).According to the score of PSQI,ISI,HAMD and HAMA,the subjects were divided into the group of chronic insomnia with anxiety and depression(CIAD),the group of only chronic insomnia(CI)and the group of healthy control(HC).The scores of MOCA,TMT,CWT and AVLT were compared in the intergroup;the coherent values of the rest status and the cognitive task status were analyzed in the intergroup and intragroup;The correlation analysis between PSQI coherent values and HAMD HAMA was performed.Result 1 The scores of MOCA decreased from HC to CI and CIAD and there were significant differences between two of three groups(P<0.05);The some CWT time of CIAD and CI were significantly longer than HC(P<0.05).The word numbers of AVLT-1 AVLT-2 AVLT-short delay in CIAD were significantly reduced compared with the HC.The word number of AVLT-2 in CI was significantly reduced than HC(P<0.05).Compared with HC,the time of Trails-A and B in the CIAD and CI were significantly longer(p<0.05).2.Compared with the HC in rest status: the delta(F3-T3 C3-T3 F4-T4 C4-T4),theta alpha-1(F3-F4 F3-T3 C3-T3 F4-T4 C4-T4),alpha-2(F3-T3 C3-T3),gamma(F3-T3 C3-T3)coherent values in CIAD and CI was significantly decreased(P<0.05)3.Compared with the HC in cognitive task status: the beta-1(F7-F8)in CIAD,the beta-1(F3-T3)in CI,the beta-2(F7-F8)in CIAD and CI,gamma(F7-F8 C4-T4)in CIDP,gamma(F7-F8 F3-T3 F4-T4)in CI coherent values were significantly decreased(P<0.05).4.The coherent values in the cognitive task status was compared with the rest status in the intragroup: the inter-hemisphere and right intra-hemispheric coherent values significantly increase in the CIAD and CI,the left intra-hemisphere coherent values significantly increase in the HC,the right inter-hemisphere coherent values in the HC significantly decrease(P<0.05).5.In the rest status of CIAD,theta(F3-T3),alpha-1(F4-T4)coherent values and HAMD had a negative correlation.In the cognitive task status of CIAD,gamma(F7-F8)coherent values and HAMA had a negative correlation.Conclusion 1.Chronic insomnia can cause a decrease in whole cognitive function,especially in cognitive flexibility,executive function,and memory.Anxiety and depression may aggravate the whole cognitive and executive dysfunction.2.The EEG coherence value can reflect the connection function of depression and anxiety related brain areas.Furthermore,it also provides an objective indicator to evaluate the degree of depression anxiety.3.The decrease of the relationship between the bilateral frontal and temporal lobe is a possible reason for the decline in whole cognitive function,executive function,and memory function in patients with chronic insomnia and anxiety and depression.4.Chronic insomnic patients with or without anxiety and depression have reduced neurological efficiency.
