Objective Based on internet of things,A “physiological-psychological-social” integration management system including remote physical monitoring,health education,individualized self-care behavioral guidance and emotional regulation was designed,in order to reduce blood pressure,increase control rate,quality of life and self-care behaviors of patients with hypertension.during the home period.Methods This study was divided into two parts.1,Investigation and analysis on home management status of hypertensive patients: questionnaire survey and semi-structural interviews were used to investigate the emotional regulation,self-care behavior and quality of life of hypertensive patients,in order to analyze the existing problems and management needs of hypertensive patients.2,Study on home-based integrated management intervention for hypertensive patients based on Internet of things: 141 satisfactory primary hypertensive patients were randomly assigned to the treatment group(n=71)and the control group(n=70))by the random numbers table.The treatment group received integration management at home,while the control group received usual internet of things management for 4 months.Before and after intervention,physiological indicators,emotional regulation scales,hypertension self-care scale of behavior and hypertension quality of life scale were selected to as outcome indicator.Results 1,Current status survey results: There was a significant difference in cognitive reappraisal and expression suppression of happiness,sadness and fear 3 emotions and total pointsin(p<0.05).There was no significant difference in cognitive reappraisal and expression suppression of aversion and anger emotions(p>0.05).Hypertensive patients had higher scores in self-care behaviors in terms of smoking cessation and alcohol restriction,medication compliance and weight management than emotional decompression,regular visits and healthy diets,and there were misunderstandings on the specific measures for self-care behavior.The order of the quality of life of hypertensive patients from the highest to the lowest was: interpersonal sensitivity,depression,anxiety,somatization symptoms,physical symptoms,work status,vitality,obsessive symptoms,hostile symptoms,sleep status and sexual dysfunction.2,There was no significant difference between the treatment group and control group on the general information and baseline outcome variables,this study was comparable.After 4 months interventions,both the treatment group and the control group witnessed improvement on physical indicators(systolic blood pressure,diastolic blood pressure,blood pressure control rate,body mass index and waist circumference),self-care behavior and quality of life,and the differences were statistically significant(p < 0.05).The scores of cognitive reappraisal,expression inhibition,and scores of all dimensions of the treatment group were significantly different from those before intervention,existing statistical difference(p<0.05).There was no significant change in emotional regulation scores of the control group before intervention(p>0.05).The treatment group increased more significantly in 7 self-care behaviors(except blood pressure monitoring),8 dimensions of quality of life(except somatization symptoms,obsessive-compulsive conditions,and work conditions)and total scores for cognitive reappraisal and expression suppression and 5 emotion scores(except happiness suppression,anger reappraisal,aversion reappraisa)than the control group,the differences were statistically significant(p<0.05).Conclusion 1,The self-care behaviors of the hypertensive patients in emotional decompression,visiting doctors regularly,healthy diet,and the quality of life in hostile symptoms,sleep conditions,and sexual dysfunction are not satisfactory.There are deficiencies in negative emotional regulation and the implementation details of self-care behavior.And it is necessary to conduct "physiological-psychological-social" integrated blood pressure management.2,Home-based integrated management on hypertensive patients based on internet of things can lower blood pressure,increase control rate,quality of life,self-care behaviors and reduce the influence of negative emotions on blood pressure.Home-based integrated management is an effective approach to contol hypertension. |
