Objective To investigate the prevalence rate,changes over time and related influencing factors of cognitive impairment in survivors of sepsis.Methods The cognitive function of survivors with sepsis admitted to the Department of Critical Care in the Tianjin Third Central Hospital were prospectively explored,and their demographic and clinical data were recorded.The cognitive function of survivors were assessed by the Montreal Cognitive Assessment at the 3rd,12 th,and 24 th months after discharge.The survivors participating in the assessment were divided into cognitive impairment group and normal group.Univariate analysis was performed to identify the risk factors of cognitive impairment,and variables that may affect the cognitive function of survivors were selected to do multivariate binary logistic regression analysis for the confirmable independence risk factors.Results(1)The prevalence rate of cognitive impairment was 61.6%(77/125),54.36%(56/103)and 54.16%(39/72)at the 3rd,12 th,and 24 th months after discharge.(2)The comparison of three scores showed that the score of the 12 th months after discharge was higher than that of 3rd months,the difference was statistically significant(P<0.05),while there was no statistically significant difference between the 12 th and 24 th after discharge.(3)Results were evaluated by univariate analysis that the age,average education years,sex,delirium duration were related to cognitive impairment in survivors of sepsis.It was showed by multivariate binary logistic regression analysis that the age [odds ratio(OR)= 1.038,95% confidence interval(95%CI)= 1.009~1.068,P = 0.01],average education years[OR=0.791,95%CI=0.677~0.923,P=0.003],delirium duration[OR=1.314,95%CI=1.002~1.724,P=0.048] were independent influencing factors of cognitive impairment.Conclusions Many sepsis survivors suffer cognitive impairment after discharge.The cognitive impairment can persist for a long time and improve over time.The average education years is a protective factor of cognitive impairment and the age and delirium duration are risk factors.
