Objective To understand the nursing needs of pregnant women with gestational diabetes mellitus and their main caregivers,and then to conduct family-centered intervention in empowerment education mode.Methods This study selected 9 cases of pregnant women with gestational diabetes mellituswho were examined and delivered in Huai'an Maternal and Child Health-Care Center during January,2017 to March,2017.By conducting in-depth interview face to face,we evaluated theknowledge of patients and their major caregivers about gestational diabetes and the situation of breastfeeding,and then foud outtheir needs of health education,and built a health education program according to their needs.The study used a convenient sampling and randomized controlled research method.Then we selected gestational diabetes mellitus pregnant women who had established card inspections in Huai'an Maternal and Child Health Hospital during March,2017 to September,2017.160 women with gestational diabetes who met the criteria were randomly divided into a control group(n=85)and an experimental group(n=91).The control group used the traditional "Honey Mommy Class" to give health education6 times,and they were free to ask questions after class between60 and 90 minutes randomly.In the experimental group,the family centered education was adopted for the health education.The pregnant women in the experimental group were divided into numberous subgroups according to the pregnancy week,and each subgroup contained 10 pregnant women.Moreover,each group includedone obstetric nurse and one midwife.One endocrinologist and another nursing graduate student were responsible for the whole process of energetic health education including 5 times of health education about glycemic control and breastfeeding.Two times of prenatal education were group study,and three times of postnatal education were performed in individual.The main contents consist of the content of blood sugar control and the nursing content of breastfeeding newborns.The health education time is arranged according to the quantity and difficulty of problems that faced by the pregnant and parturient and their main caregivers.and the time is controlled between 60 and 90 minutes.Two groups of pregnant women were provided with free blood glucose test and then the blood sugar level of each women was recorded.The maternal blood glucose control,breastfeeding self-efficacy and breastfeeding rate,the social understanding and the ability of self-care were compared between the two groupsResults1.Most gestational diabetes pregnant women and their main caregivers lack knowledge about blood glucose control and breastfeeding.2.The self-care compliance of pregnant women with gestational diabetes is susceptible to the influence of caregivers based on their cognition and attitude of gestational diabetes mellitus and breastfeeding.3.Before the intervention,there was no statistically significant difference between the two groups' general maternal data,breastfeeding self-efficacy,perceived social support and self-care ability,thereforethese data were comparable.After the intervention,the blood glucose control of pregnant and lying in pregnant women in the experimental group was better than that of the control group,and the difference was statistically significant(P< 0.05).Furthermore,the scores of breastfeeding self-efficacy,perceived social support,self-care ability and breastfeeding rate in the experimental group were statistically higher than that of the control group(P< 0.05).Conclusions1.The majority of gestational diabetes mothers and their main caregivers lack knowledge about glycemic control and breastfeeding.2.The support of the family not only affects the blood sugar control of pregnant and parturient women in gestational period,but also affects the behavior of breastfeeding.It is imperative to bring pregnant and parturient women and their main caregivers into health education.3.The application of family centered health education will help to improve the control of blood sugar and improve the level of self-efficacy of breastfeeding,the rate of breastfeeding,the social understanding and the ability of self-care.
