A Study Of The System Of Hospital Information Public Reporting In America And China

[Purpose]The purpose of this study is to summarize the practical experience of hospital information public in the United States,to analyze the elements of the hospital information public system,to construct the framework model of the hospital information public system in the United States,so as to provide theoretical support and evidence-based basis for the promotion and improvement of China’s hospital information public system.[Methods]1.Using the method of literature review,searched thousands of related literature from the domestic and foreign major databases and organizations’ websites,581 literatures were screened out by duplicate checking and reading the abstracts,and then 126 papers were selected by in-depth reading.2.Using the interest group analysis,analyze the roles played by the interest groups in the public reporting of hospital information,as well as their respective positions and rights.The policy maker4.0 software was used in this part.3.Using the logic reasoning method,guided by the thought of system theory,the theory of epistemology,system theory,communication science,management and other related theories are integrated.Converted the five elements of the dissemination process of the Larswell model(the "five W" model)to the five elements of hospital information dissemination,then integrated the DADS theoretical model,and last constructed the theory of hospital information public system model.[Results]1.For the public subject,there exist strong relationships between the government,third-party organizations and hospitals of the US.The government not only makes public policies,but also builds a unified and open platform.The third party organizations public to the specific scope and is a major supplement to the government.The hospitals are supervised by the government and the third party organization,paying more attention to its own competitiveness and the reliability of the information.2.The content of public hospital information between China and the US has similar places,but more is difference.Their structural indicators are both relatively rich,but China lack of medical service quality indicators and patient experience indicators.3.The interest groups of America’s hospital information public system mainly include governmental organizations,employers / buyers,hospitals and patients.Among them,the government control a large number of medical resources,and through legislation and economic incentives to promote the hospital information public policy,while monitoring the hospital’s information public behavior;as well as establish a national hospital information public platform;The hospitals own original hospital information,they public hospital information actively can help to reduce the government’s regulatory costs and improve regulatory efficiency.So the government and hospitals play a decisive role in the hospital information public.Employers / buyers and health insurers have a lot of money,combined with third-party agencies with intellectual resources such as intellectual resources,can involve more social resources,reduce government financial pressure,and make more objective reviews of public information.The Patients under information asymmetry make the government feel public pressure,so to promote government and hospital public information.[Conclusions]The framework model of hospital information public system of Chain and America is similar,both including the information transmission loop,that is,the hospital information transferred from the public subject to the public object,then the public object feedback to the public subject,and the DADS model of public-analysis-open-punish.But the elements of the two systems have many differences.China should learn from the experience of the United States,design comparable and understandable public indicators in accordance with scientific standards,.Build an authoritative and comprehensive hospital information public platform.The government should combine the actual situation of our country,strengthen the policy commitment,announce more targeted policies and regulations,take specific incentive measures to encourage hospitals to public information.Hospitals should strengthen public awareness,train health care workers,and bring in information technology professionals,control of information sources strictly to ensure the accuracy of public information,and enrich the content of public reporting information.Government,hospitals and academic organizations and other third-party institutions should strengthen cooperation to push the public reporting of hospital information forward.
