| Objective:Lower urinary tract symptoms(LUTS)is a common disease in female that easily lead to life problems and affect mental health.LUTS has a variety of clinical manifestations,including nocturia,frequent urination,urgency,dysuria,and urinary incontinence.Due to the lack of awareness of LUTS,some LUTS symptoms are often hidden and unintelligible,and they are easily overlooked,resulting in a low rate of disease visits.Although LUTS is not a fatal disease,it seriously affects the quality of life of patients,and even leads to social barriers.At present,most of them are about the etiology research of male LUTS,while female LUTS’s etiology research reports are few.In recent years,female pelvic floor dysfunction has gradually attracted widespread attention from scholars at home and abroad.Studies have pointed out that pregnancy delivery is closely related to the occurrence of female pelvic floor dysfunction,and the relationship between pregnancy delivery and stress urinary incontinence is particularly prominent.However,there are fewer studies on the relationship between pregnancy delivery and female LUTS,and there are fewer studies on the related risk factors.China is a country with a large population.By 2016,the total population has reached 1.4 billion.The number of women giving birth every year during the childbearing period is large,which means that the situation in which women face LUTS problems in the future will become increasingly prominent.Therefore,it is of great practical s ignificance to understand the prevalence of various types of LUTS in pregnant women,to explore their related risk factors and to study the impact on the quality of life.Research purposes include:1、To study the prevalence and trend of various types of LUTS in primiparous women at three stages of gestation;2、To study the related risk factors of LUTS in primiparous women during pregnancy,so as to provide clinical data for making preventive measures of LUTS and improve the health consciousness of female pelvic floor function;3、To study the influence of various types of LUTS on the quality of life of primiparous women during pregnancy,aiming to arouse the attention of clinicians and enhance the science awareness of female LUTS.Method:In this study,a questionnaire survey was used to collect the Chinese version of the International Consultation on Incontinence Questionnaire Lower Urinary Tract(ICIQ-FLUTS).The questionnaire was collected from May 2017 to December 2017 at the Third Affiliated Hospital of Guangzhou Medical University.The statistical software was used to analyze the three stages of gestation.The prevalence and trend of various types of LUTS in primiparous women were studied,and Logistic regression analysis was used to investigate the risk factors of LUTS in primiparous women during pregnancy,and to understand the effect of various types of LUTS on the quality of life of primiparous women during pregnancy.Result:A total of 980 valid questionnaires were returned for this project,with a pass rate of 96.1%and an average age of(29.6±4.5)years.In the first trimester(<13+6weeks),there were 320 cases,and the average number of gestational weeks was(7.7±2.2)weeks.In the second trimester(<14-28weeks),there were 316 cases,and the average number of gestational weeks was(22.5±4.2)weeks.In the third trimester(≥28weeks),there were 344 cases,and the average number of gestational weeks was(35.2±4.0)weeks.1、In the three stages of gestation,storage symptoms was the mainly LUTS type.Frequency was the most common type of LUTS,and the most rare type of LUTS was sex leaking urine.In the third trimester,the prevalence rate of storage symptoms LUTS was as high as 99.1%,of which the prevalence of frequency was 88.1%.2、In the composition ratio of urinary incontinence,SUI is the most common type of urinary incontinence in primiparous women during pregnancy,followed by UUI and MUI.3、The prevalence of storage symptoms LUTS in primiparous women during pregnancy was increased with the number of weeks of gestation.In the three stages of gestation,the prevalence of nocturia,urgency,and MUI rises linearly,while the prevalence of frequency,SUI and UUI increased in the early pregnancy,and gradually became stable after reaching the middle of pregnancy.4、The number of gestational weeks,age,pre-pregnancy BMI,number of fetuses,abortion and GDM were all related to the occurrence of LUTS in primiparous women during pregnancy,and they were all risk factors.But there was no significant correlation with voiding symptoms.Among them,the history of abortion was only related to the occurrence of SUI,and GDM is only associated with the occurrence of nocturia.5、In the early stage of gestation,all types of LUTS are mainly mildly manifested.In the middle and later stages of gestation,frequency and nocturia are mainly moderate-severe,and other types of LUTS remain mild as the main performance.6、The score of ICIQ-FLUTS questionnaire was lower in the early-pregnant women,and 96.3%in<10 points.The distribution of wome n’s scores in the middle of pregnancy was changed,and 88.0%scored<10 points,and 11.4%scored between10-20 points.By the end of gestation,the highest score was 45,the proportion of<10points decreased to 82.3%,and the proportion of 10-20 was 14.8%,and the proportion of 20 or more was 2.8%,of which more than 40 points accounted for 0.6%.Conclusion:1、The prevalence of various types of LUTS in primiparous women during pregnancy is very common.The storage symptoms LUTS is the main type of LUTS,and its prevalence is increasing with the increase of gestational weeks.Urinary frequency is the most common type of LUTS.2、The prevalence of LUTS in primiparous women during pregnancy may be the result of multiple factors.The number of weeks of gestation,age,pre-pregnancy BMI,number of fetuses,abortion history,and GDM were all related risk factors for LUTS.But there was no significant correlation with voiding symptoms.Among them,the history of abortion was only related to the occurrence of SUI,and GDM is only associated with the occurrence of nocturia.3、In the early stage of gestation,all types of LUTS were characterized by mild manifestations.To the middle and late stage of gestation,frequency and nocturia were mainly medium to severe,and the other types of LUTS were still mild.With the increase of gestational weeks,the higher the ICIQ-FLUTS questionnaire score for primiparous women.The more serious the problem is,the more the quality of life is affected. |
