Study On The Key Elements Of The Formation Of Malignant Tumor Subgroup Model

BackgroundMalignant tumor is a typical serious disease with a long diagnosis and treatment cycle and high costs.As the aging process accelerates,malignant tumor incidence and death rate keep increasing.The increasing costs of malignant tumors have a great impact on the medical insurance fund and cause a heavy economic burden on the country,society and individuals.The disease payment system has been proved by international experience that the effective cost control can also improve the medical quality and efficiency.It is the focus and the difficult point of China’s current medical insurance payment reform.The related studies have shown that push press disease to the payment system in China is so slow,the development is not ideal.Research on malignant tumor diseases group is still at the preliminary exploration stage,it is not applied in most areas.Under the new situation,it is very important and urgent to explore the key elements of the disease classification pattern from the perspective of malignant tumor.ObjectiveThrough in-depth to explore the factors affecting the hospitalization cost of malignant tumors and key elements of disease group,is a pattern for government departments to build a group of malignant tumor diseases and improving macro-reforms system to provide decision basis of establishing the classification model of malignant tumor diseases and improving the disease payment system,but also can provide countermeasures and suggestions for medical institutions to actively adapt to the disease payment reform.ContentsBased on the research status at home and abroad literature,the thesis aims to analyze the characteristics of the different payment hospitalization expenses and fees influence factors on the basis of malignant tumor,lung cancer,thyroid cancer,liver cancer,colon cancer,nasopharyngeal carcinoma,five kinds of malignant tumor medical record home page data grouping analysis,and to explore the key factors and countermeasures for the classification of malignant tumor diseases.MethodsUsing the literature research,statistical methods(including descriptive statistics,nonparametric test,multivariate logarithm linear stepwise regression analysis,chi-square automatic interaction test(CHAID),etc.,with P<0.05 for the difference was statistically significant)and depth interview method for study.Results(1)Whether surgery,accompanying diseases and other factors are important nodes for the classification of malignant tumor diseases;(2)There are certain differences between the grouping nodes of different regions and subdivided diseases;(3)Reasonable and feasible results of disease classification using decision tree;(4)The number of hospitalized patients with malignant tumors and the increase of expenses are severe.Conclusions(1)Treatment methods,conditions,regions and subdivided diseases should be included in the key elements of the classification of malignant tumor diseases;(2)The decision tree method can be used to construct the classification model of malignant tumor diseases.(3)The payment and management of deviation cases is an important part of the grouping payment of diseases;SuggestionsFor the government,combined with the particularity of malignant tumor diseases,it can build up a subgroup payment system according to local conditions.(1)The reform of disease seed payment should focus on the promotion of malignant tumor;(2)Use statistical methods to focus on the key elements of disease classification to build up the disease classification model;(3)Establish and improve the case evaluation mechanism,formulate the principle of payment in a scientific and reasonable manner;(4)The implementation of grouping payment for diseases should be done in the costs accounting of diseases simultaneously;For the hospital,we can strengthen the connotation management of hospitals by improving the quality and reducing the consumption of diseases.(1)Consolidate the payment infrastructure for diseases and enhance the quality of information on the first page of medical records;(2)Strengthen the information-based management of clinical pathway and standardize the diagnosis and treatment service of malignant tumors;(3)To build up day medical center and MDT diagnosis and treatment model to improve the diagnosis and treatment efficiency of malignant tumors.InnovationsThis paper mainly focus on the costs of malignant tumor grouping method of factors analysis and explore diseases and charge standard,focus on the key elements of malignant tumor in different patient group and Group differences of different subdivided diseases and giving suggestions.
