purpose : To investigate the value of CT and PET-CT in preoperative staging of esophageal cancer.Methods: Retrospectively collected preoperative PET-CT data from 37 patients with esophageal cancer who underwent surgical treatment of esophageal cancer in Shandong Cancer Hospital from 2012 to 2017,and from 2015 to 2017,the esophageal cancer radical surgery was performed at Shandong Cancer Hospital.Preoperative CT examination data,PET-CT examination data,surgical records and postoperative routine pathology of 184 patients with esophageal cancer were performed for PET-CT N-segmentation,CT and TN staging of postoperative pathology,and CT and PET were performed.-The staging results of CT were compared with the staging results of postoperative pathology.SPSS17.0 was used to process the data.The differences between the two groups in terms of staging between the groups,PET-CT and CT examination methods were analyzed using chi-square test.P<0.05 indicated that the difference was statistically significant.Results: The accuracy of CT on T staging was 66.8%,and the accuracy of T0,T1,T2,T3,and T4 was 50%,38.1%,56.5%,82.9%,and 19.0%,respectively.The sensitivity to T staging was: The specificity,positive predictive value,and negative predictive value were 95.3%,66.7%,99.5%,and 18.1%,respectively.The accuracy of CT on T3 staging was higher than that of other staging,and the difference was statistically significant(X2=64.47,P<0.05).The accuracy of CT in the diagnosis of esophageal cancer N staging was 68.5%,and the diagnostic accuracy of NO,N1,and N2 were 73%,78.9%,and 25%,respectively.The sensitivity,specificity,and positive pred ictive value of N staging for esophageal cancer were analyzed.The negative predictive values were 84.8%,88.8%,79.8%,and 84.0%,respectively.The diagnostic accuracy of CT for N0 and N1 was significantly higher than that of N2,and the difference was statistically significant(X2=16.39,P<0.05;X2=20.49,P<0.05).The accuracy of PET-CT on N stage was 60.0%,and the sensitivity,specificity,positive predictive value,and negative predictive value were 86.7%,66.7%,76.5%,and 80%,respectively.The accuracy rate of CT for N staging was 68.5% higher than that of PET 60%(X2=0.958,P=0.328>0.05),so the difference was not statistically significant,and CT and PET could not be considered different for N staging test efficiency of esophageal cancer.Conclusion: CT has higher sensitivity to TN staging of esophageal cancer,and CT has higher accuracy for T3 staging and N0-1 staging.CT has higher accuracy for N staging than PET-CT,and the difference is not statistically significant..A number of articles have reported that the accuracy of PET-CT for N-stage is higher than that of CT.The inconsistent results of this study may be due to the small number of samples. |
