| Purpose:To observe the clinical curative effect of Qigui Tongluo Granules treating diabetic peripheral neuropathy patients and the expression of NF-κB as well as IкBα on the mRNA expression of the sciatic nerve in rats.To probe into the mechanism of Qigui Tongluo Granules improving the treatment of diabetic peripheral neuropathy.Method:1 Clinical studyThe 64 patients were diagnosed with diabetic peripheral neuropathy,who were in hospital outpatient and ward of Anhui University of traditional Chinese Medicine First Affiliated Hospital from June 2016 to June 2017.They were randomly divided into control group and treatment group of 32 cases.They were received diabetes education,kept diet,did exercise and lowerde blood fat.The control group took Mecobalamin Tablets,besides,the treatment group took Mecobalamin Tablets and Qigui Tongluo Granules.The course of treatment is 12 weeks.Record TCM syndromes score,blood glucose and lipids,Toronto score,nerve conduction speed,tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)being detected by ELISA method of serum samples.They were compared before and after treatment in order to evaluate the clinical efficacy.2 Animal experiment65 male healthy SD rats were fed with adaptive feeding for a period of 1 week.10 rats were set as blank group by random drawing.The remaining 55 were moulded.The rats were set as the success of the DPN modeling symbol,with extending the plantar thermal response time and slowering of nerve conductive velocity.According to the random principle,the successfully established rats models were randomly divided into the following groups,including the model group(n = 11),the Western medicine group(n = 10),the high-dose group of Chinese medicine(n = 10)and the low-dose group ofChinese medicine(n = 10).The blank group and model group were given normal saline orally.The Western medicine group were given Mecobalamin Tablets by gavage.The high-dose and low-dose group of Chinese medicine were given different doses of Qigui Tongluo Granules.After 12 weeks treatment,general conditions of rats were recorded.Detect the plantar thermal response time and nerve conduction velocity of rats.Collecte blood sample of rats.Use ELISA to detect tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in serum samples.Separate sciatic nerve tissue.Use Western blot to detect the protein levels of NF-κB and IкBα.Apply RT-PCR to detect the expression of NF-κB mRNA and IкBα mRNA.Result:1.Clinical findingsThe total effective rate 87.10 %,in the control group total effective rate 63.33%,the difference was statistically significant(P<0.05).Compared within groups : the scores of TCM syndromes score was lower in treatment the group(P<0.05).Toronto scores and the levels of FPG were significantly lower,The level of nerve conduction velocity was higer,The levels of TNF-α and IL-6 were lower than before.The difference was statistically significant(P<0.05 or P<0.01).Compared between two groups: tn the treatment group the scores of TCM syndromes score,Toronto score and the levels of FPG were significantly lower.The level of nerve conduction velocity was higer.The levels of TNF-α and IL-6 were lower than in the control group The difference was statistically significant(P<0.05 or P<0.01)2 Animal experiment resultsCompared with the blank group: the body weights of rats reduced,the blood glucose increased,the plantar thermal response time extended and nerve conduction velocity reduced,the levels of TNF-α,IL-6,NF-κB,IкBα and their mRNA increased in the model group.The difference was significant(P< 0.05 or P<0.01).Compared with the model group:The the plantar thermal response time shortened and nerve conduction velocity increased,The levels of TNF-α,IL-6,NF-κB,IкBα andtheir mRNA decreased in the Western medicine and high-dose group of Chinese medicine.The difference was significant(P < 0.05 or P < 0.01).The blood glucose was lower in the high-dose group of Chinese medicine rats.(P<0.05).In the low-dose group of Chinese medicine,the the plantar thermal response time shortened and nerve conduction velocity increased,The levels of TNF-α and IL-6 were decreased.The difference was significant(P < 0.05 or P < 0.01).Conclusion:Qigui Tongluo Granules can improve the treatment of diabetic peripheral neuropathy and increase nerve conduction velocity.It maybe works through reduce the expression of NF-κB,IκBα and their mRNA,as well as the release of Inflammatory factor.These changes reduce inflammatory injury in the body and tissues. |
