Objective :To summarize the rule of lymph node metastasis of peripheral type non-small cell lung cancer in T1 stage(tumor diameter less than 3cm)and to discuss the reasonable operation plan.Method:The clinical data of 254 patients with peripheral NSCLC were analyzed retrospectively from October 2012 to December 2017.There were 117 males and 137 females,aged between 39 and 75.The number of lymph node dissection and postoperative pathology confirmed the lymph node metastasis.Result:A total of 4,580 lymph nodes were dissected in the whole group,with an average of 18 in each patient.146 lymph nodes were confirmed to be metastatic after surgery,with a metastatic rate of 3.18%.Postoperative pathological examination showed no lymph node metastasis in 212 patients.There were 12 cases of lymph node metastasis in groups 12,13 and 14 with a metastatic rate of 4.72%.The rates of lymph node metastasis in T1 a,T1b and T1 c were 0%(0/2185),3.06%(49/1600)and 12.20%(97/795),respectively.Lymph node metastasis rate of solid tumor group was6.36%(126/1983),and that of frosted nodule group was 0.77%(20/2597).The rate of lymph node metastasis in the pleural indentation group was 4.86%(120/2466)and 1.23%(26/2106)in the non-pleural indentation group.Lymph node metastasis rate in adenocarcinoma group was 3.26%(127/3895)and 2.76%(9/325)in squamous cell carcinoma group.Conclusion:The metastasis rate of peripheral NSCLC lymph nodes at stage T1 increased with tumor diameter,tumor solid components and pleural invasion.Different surgical procedures were selected according to tumor diameter,tumor solid composition and pleural invasion. |
