1 ObjectiveCholecystolithiasis,cholecystitis is one of the common diseases of surgery,with high morbidity and serious impact on human health.Several studies have shown that the repeated physical and chemical stimulation of calculi and inflammation can induce calculi and even cancer.TCM has a significant advantage in treating gallbladder stones and cholecystitis.We conclude from the experience of the discipline that the treatment of “biliary diseases are treated based on the liver”.Shugan Liqi,nourishing the liver and Qinggan Lishi were used to treat gallbladder stones.At the same time,the twelve kinds of Shugan Lidan granule was developed according to the Shugan Liqi.The effect of the twelve kinds of Shugan Lidan granule on gallbladder inflammation in patients with gallbladder stone by clinical observation.Furthermore,the effects of Shugan Liqi and nourishing the liver、Qinggan Lishi on gallbladder inflammation in guinea pigs were compared.Objective evaluation the twelve kinds of Shugan Lidan granule inhibits gallbladder inflammation effect.To sum up,we conducted the following research.2 Methods2.1 Clinical observationChoose 30 cases that meet the criteria,randomly divided into control group and study group 15 cases.Control group:(1)routine surgical preparation after hospitalization,laparoscopic cholecystectomy.(2)after the operation,the routine support therapy,such as rehydration,anti-infection,and protection of liver function.The study group: on the basis of the control group,we increased twelve kinds of Shugan Lidan granule before and after operation.Using automatic blood cell analyzer test two groups of patients with preoperative(hospital)and after the treatment(5 days after the surgery)in serum WBC 、N %concentration;Using ELISA method to detect the Serum CRP levels in both groups before surgery(hospital)and after treatment(5 days after surgery).Using optical microscope observing the pathologic conditions of gallbladder tissue.The results are analyzed statistically.2.2 Animal experiment75 guinea pigs,male 37 and female 38,quality(260 + + 20)g,randomly divided into 5 groups: normal group(15),model group(15),shugan group(15),yanggan group(15),and qinggan group(15).Normal group was given normal animal feed,normal vegetable quantity,and the same amount of normal saline irrigation;the model group was given high fat and cholesterol induced stone of guinea pigs food,a small amount of vegetables and the same amount of physiological saline irrigation;the shugan group was given high fat and cholesterol induced stone of guinea pigs food,a small amount of vegetables and at the same time are gavaged with twelve kinds of Shugan Lidan granule;the yanggan group was given high fat and cholesterol induced stone of guinea pigs food,a small amount of vegetables and at the same time are gavaged with Yi Guan decoction granule;the qinggan group was given high fat and cholesterol induced stone of guinea pigs food,a small amount of vegetables and at the same time are gavaged with Longdan Xiegan granule.After 8 weeks of feeding,observing the growth of guinea pigs and calculating the formation rate of calculus;Using infrared spectroscopy qualitative method detecting the stone components;Using optical microscope observing the pathologic changes of gallbladder and liver;Using automatic blood cell analyzer detecting the WBC and N concentrations in guinea pig serum;Using ELISA method to detect the concentration of MUC1 、 CRP 、 TNF-α and IL-6 in guinea pig bile.The results are analyzed statistically.3 Results3.1 Clinical observation3.1.1 The WBC and N% levels in the serum were significantly different in the 5 days after the operation of the control group and the study group(p﹤0.01).Compared with the control group,the WBC and N% levels in the serum were significantly reduced in the 5 days after surgery,with statistically significant differences(p﹤0.01).The experimental results showed that the twelve kinds of Shugan Lidan granule had the effect of reducing WBC and N% level in serum.3.1.2 The CRP levels in the serum were significantly different in the 5 days after the operation of the control group and the study group(p﹤0.01).Compared with the control group,the CRP levels in the serum were significantly reduced in the 5 days after surgery,with statistically significant differences(p﹤0.05).The experimental results showed that the twelve kinds of Shugan Lidan granule had the effect of reducing CRP level in serum.3.1.3 Compared with the Control group,the degree of gallbladder inflammation of Study group is improved significantly.3.2 Animal experiment3.2.1(1)Compared with the model group,the stone formation rate of Shugan group,Yanggan group and Qinggan group guinea pigs is significantly decreased(all P<0.01).(2)Cholesterol stone is the main stone in the formation of stones.3.2.2(1)Compared with the model group,the degree of hepatic steatosis of Shugan group,Yanggan group and Qinggan group guinea pigs is improved significantly.(2)Compared with the model group,the degree of gallbladder inflammation of Shugan group,Yanggan group and Qinggan group guinea pigs is improved significantly.3.2.3 Compared with the model group,the concentration of MUC1 in bile of Shugan group,Yanggan group and Qinggan group guinea pigs are significantly lower(all P<0.01).Compared with normal group,the concentration of MUC1 in bile of Shugan group,Yanggan group and Qinggan group guinea pigs are significantly increased(all P<0.01).Compared with Shugan group,the concentration of MUC1 in bile of Yanggan group and Qanggan group guinea pigs are significantly decreased(all P<0.05).3.2.4 Compared with the model group,the concentration of WBC 、 Nin serum of Shugan group,Yanggan group and Qinggan group guinea pigs are significantly lower(all P<0.01).Compared with normal group,the concentration of WBC、N in serum of Shugan group,Yanggan group and Qinggan group guinea pigs are significantly increased(all P<0.01).There was no significant difference of WBC 、 N in serum between Shuggan group,Yanggan group and Qinggan group(all P﹥0.05).3.2.5 Compared with the model group,the concentration of TNF-α、IL-6、CRP in serum of Shugan group,Yanggan group and Qinggan group guinea pigs are significantly lower(all P<0.01).Compared with normal group,the concentration of TNF-α、IL-6、CRP in serum of Shugan group,Yanggan group and Qinggan group guinea pigs are significantly increased(all P<0.01).There was no significant difference of TNF-α、IL-6、CRP in serum between Shuggan group,Yanggan group and Qinggan group(all P﹥0.05).4 Conclusion4.1 For patients with acute cholecystitis and cholecystolithiasis,the twelve kinds of Shugan Lidan granule can reduce the level of WBC、N% and CRP levels in serum,and reduce the body’s inflammatory response.This may be one of the main mechanisms for the development of gallbladder stones and gallbladder inflammation.4.2(1)Twelve kinds of Shugan Lidan granule,YiGuan decoction granule and Longdan Xiegan granule can reduce the effect of gallbladder stone formation in guinea pigs by controlling the inflammatory response of gallbladder.(2)Twelve kinds of Shugan Lidan granule,Yi Guan decoction granule and Longdan Xiegan granule have the effect of inhibiting the hepatic steatosis and reducing the inflammatory response of gallbladder in guinea pigs.The effect of Yi Guan decoction was better than Twelve kinds of Shugan Lidan granule and Longdan Xiegan granule in guinea pig hepatic steatosis.(3)Twelve kinds of Shugan Lidan granule,YiGuan decoction granule and Longdan Xiegan granule can reduce the MUC1 in guinea pigs bile.The MUC1 can reverse the formation of stony bile,inhibiting the formation of stones.(4)Twelve kinds of Shugan Lidan granule,Yi Guan decoction granule and Longdan Xiegan granule can reduce the level of WBC、N in guinea pigs serum,they can inhibit the formation of stones by reducing the inflammatory response of gallbladder.(5)Twelve kinds of Shugan Lidan granule,Yi Guan decoction granule and Longdan Xiegan granule can reduce the level of CRP、TNF-α and IL-6 in guinea pigs serum,they can inhibit the release of inflammatory cytokines and relieve inflammation in guinea pigs.And the three has the effect of inhibiting the formation of stones.
