| Objective:The objective of this study is to investigate the prehospital delay and to analysis its associated factors of the patients with acute ischemic stroke in Yan’an area.Methods:A questionnaire survey was conducted on a total of 240 patients with acute ischemic stroke who consulted the Department of Neurology,Yan’an University Affiliated Hospital from October 2015 to December 2016.These patients were divided into two groups according to their pre-hospital delay time: delay ≤3.5h group and delay >3.5h group.The questionnaire included personal basic information,prehospital delay time,prehospital delay factors.A logistic regression analysis was conducted on these prehospital delay factors.Results:Among the total 240 patients,223 patients met the requirement in this study,including the delay ≤3.5h group of 81 cases(36.3%),delay >3.5h group of 142 cases(63.7%)and thrombolysis of 4 cases(1.8%).1.The factors to shorten the pre-hospital delay time:(1)the onset is in a daytime;(2)the symptoms are found by others;(3)the earliest symptoms includes visual changes,consciousness changes and barylalia(barylalia≥2);(4)the symptoms are cerebrovascular disease which is known;(5)the transport method is 120 ambulance;(6)the earliestly visited hospital of grade is one or two.The factors to extend the pre-hospital delay time:(1)the earliest symptoms are limb weakness,numbness or headache;(2)thetransport methods are private car,taxi and so on.2.The binary logistic regression analysis of the factors affecting the pre-hospital delay time confirmed that the positive factors of the arrival of the hospital within 3.5hours is that the firstly symptom is conscious changes.The negative factor is that the symptom is headache.Conclusion:1.Patients and their families are extremely lack of the knowledge of acute ischemic stroke.,and the urgency and benefits of thrombolytic therapy are rarely known,which resulte the pre-hospital delay.2.The knowledge of acute cerebrovascular disease and the early thrombolytic therapy of acute ischemic stroke,and the correct way to deal with the disease should be popularized to shorten the prehospital delay;The primary hospital medical technology should be strengthened to improve the early thrombolysis rate,thereby reducing morbidity and mortality as well as improving the rate of survival and the quality of patients’ life. |
