ObjectiveTo identify whether the internetwork functional network connectivity of resting state is changed between healthy controls and medial temporal lobe epilepsy(MTLE)patients,including right mesial temporal lobe epilepsy(RMTLE)and left mesial temporal lobe epilepsy(LMTLE),with the independent component analysis(ICA).Materials and methods19 LMTLE patients and 14 RMTLE patients,confirmed by surgery pathology,were selected and divided into two groups,and a control group consists of 32 healthy volunteers with corresponding age,gender and education level.3D high resolution images,T1 anatomy images and resting state fMRI images were obtained on Siemens Trio 3.0 T magnetic resonance scanner.DPARSF and SPM8 based on Matlab were utilized to preprocess the resting state fMRI data,and the process including:file conversion,time correction,head movement correction,spatial standardization and spatial smooth.Using GIFT to extract resting-state brain networks(RSNs),then 30 independent components were obtained,from which 7 RSNs and their average time sequences were selected and compared in pairs respectively to calculate the correlation,namely the functional connection(FC)between networks.First,calculated Pearson correlation coefficient of FC between two networks within each group,then to Fisher'z transformation.Every z value was tested by one-sample t-test to determine whether FC of each pair was statistically significant,and those had statistical significances were compared between groups.Results1.Compared with the healthy controls,the FC between central executive network(CEN)and the anterior region of default network(aDMN)(P<0.001),the posterior region of default network(pDMN)and left lateral frontoparietal network(1FPN)(P=0.004),pDMN and the right lateral frontoparietal network(rFPN)(P=0.005),CEN and dorsal attention network(dATN)(P=0.001),dATN and rFPN(P<0.001)are changed in LMTLE group,among which the positive function correlation of dATN-CEN,dATN-rFPN reduce,but that of CEN-aDMN,pDMN-1FPN,pDMN-rFPN increase;2.Compared with the healthy controls,in RMTLE group,the FC between the anterior region of default network(aDMN)and CEN(P<0.001),pDMN and CEN(P=0.046),pDMN and rFPN(P=0.003),dATN and rFPN(P<0.001),CEN and salience network(SN)(P=0.022)are changed when compare with HC,among which with the positive FC of aDMN-CEN,pDMN-CEN,pDMN-rFPN,SN-CEN are increasing,while the positive FC of dATN-rFPN is decreasing;3.There is no significant statistical difference(P>0.05)of FC between RMTLE group and LMTLE group.ConclusionCompared with HC,changes of FC in MTLE patients mainly concentrated on DMN,FPN,dATN and CEN.It suggests that changes of FC may be related to the brain networks' reconstruction,and can explain part of the clinical symptoms and pathological physiology of MTLE patients.
