Research On Service Capability Of County Hospitals In Hubei Province Under The Hierarchical Medical System

Currently,the contrast of the needs for tertiary hospitals and basic health agencies is still exist and hard to be relieved,leading to huge waste in health resources.The establishment of the Hierarchical Medical System suitable to our country is crucial to the new medical reform and also essential to the modernization of governance system for public hospitals.During the process,the Hierarchical Medical System proposes a new and further requirement of promoting service ability for county-level public hospitals,as they play the leading role for both urban and rural medical health systems,especially for the primary diagnosis in patients.How to elevate the comprehensive service capabilities and strengthen the construction of clinical specialist for county-level public hospitals by the aid of the current county-level public hospital reform is the main content discussed herein.The goal of this research is to propose relevant strategies of improving county hospitals’ service abilities taking Hubei province as example on the basis of their function position in the Hierarchical Medical System.Taking attitude of problem-solving orientation,on the basis of literature research,this paper thus constructing theoretical framework of service ability of county hospitals,which includes abilities of obtaining resources,medical services as well as information and technology construction.Under the theoretical framework,this research employs the methods of data collection,descriptive analysis and comparative analysis to introduce resource acquisition ability containing capital,housing,operation rooms,beds,technicians and equipment and medical service abilities including outpatient amount,hospitalization,the usage of the bed and doctors’ working efficiency in Hubei county hospitals.The aim is to find the deficiency in service ability of county hospitals in Hubei province and propose corresponding countermeasures and suggestions for improvements for the sake of future policy making and reference for county-level public hospitals to undertake the establishment of the Hierarchical Medical System.This study found that the current status of hospital services in Hubei Province is as follows:First,the capability of resources acquisition such as the national investment,fixed assets and current assets,human resources,business space,operating room,beds,equipment,is strengthening;the service volume involving outpatient amount,the number of admissions,the number of hospital discharge,the usage of hospital beds and physician work efficiency have improved to a certain degree.Meanwhile,there has been an imbalance in manpower structural and a shortage in high-quality talents;together with relatively low level of information construction and telemedicine in county hospitals,which makes the long-distance medical treatment remains to be exploitated;the construction of clinical specialists is still in infancy.According to the lack of service capacity of county hospitals in Hubei province,this study thus putting forward policy recommendations as follows:firstly,to establish the talent introduction and incentive mechanism to enhance clinical performance;secondly,to enhance the level of information and telemedicine coverage;thirdly,to strengthen the construction of clinical specialists for improving scientific research capacity;fourthly,to promote the vertical integration of medical resources to enhance clinics level comprehensively;fifth,to carry out the delicacy management and the cultural connotation construction overall in county hospitals.
