| Objective To investigate the significance of serum-ascites albumin gradient(SAAG),serum prealbumin(PA),total bile acid(TBA),cholesterol(CHO)and prothrombin activity(PTA)in assessment the severity of decompensated liver cirrhosis patients,treatment effect and prognosis.Methods A retrospective analysis with 142 patients,including 37 cases of liver failure,29 cases of decompensated cirrhosis Child B,39 cases of decompensated cirrhosis Child C,from September 2014 to July 2015 to department of infectious diseases in our hospital and 37 cases who went to medical examination center to get physical examination.Recording the result of their laboratory blood tests and imaging examination to calculate Child-Pugh and model for end-stage liver disease in serum sodium(MELD-Na).Then grouped the patients according to the situation.Experimental data were processed by SPSS,using T test,Kruskal-Wallis test,mono factor analysis of variance,Kruskal-Wallis H test,Pearson and Spearman correlation analysis,stepwise multiple regression analysis and Logistic single factor and backward stepwise mutifactor regression analysis to discuss about the significance of SAAG,PA,TBA,CHO and PTA in decompensated liver cirrhosis patients.Result SAAG is closely related to the portal pressure and liver damage severity,and have obvious influence on Child-Pugh.SAAG combine MELD-Na has significant difference(P < 0.05)to single MELD-Na in judging curative effect and prognosis to the ascites patients with cirrhosis.PA,TBA,CHO and PTA have statistical significance difference(P < 0.05)according to the degree of the liver damage and curative effect to divided into groups.PTA,TBA and PA have obvious influence on Child-Pugh,PTA have the same to MELD-Na.In addition,the PA,TBA,CHO and PTA combine Child-Pugh and MELD-Na has significant difference(P<0.05)to single Child-Pugh and MELD-Na in judging curative effect and prognosis to decompensated cirrhosis.Conclusion SAAG is reliable to reflect the portal vein pressure,are closely associated with portal hypertension caused gastrointestinal bleeding.Detecting the level of SAAG,PA,TBA,CHO and PTA can sensitively,rapidly and specifically reflect the degree of liver damage.Besides,this can accurately determine the disease and the clinical curative effect and prognosis. |
