A Comparative Study On The Antiplatelet Therapy Of Ticagrelor And Clopidogrel

Objective To observe the antiplatelet efficacy of ticagrelor or clopidogrel in the patients with acute coronary syndrome(ACS)treated with dual antiplatelet drug by using thrombelastography(TEG)for detecting platelet Inhibition rate.Methods Patients with suspected"coronary heart disease"from June,2014 to June,2015 in Department of Cardiology,my Hospital.Based on clinical symptoms,combined with ECG,Serum markers of myocardial necrosis and coronary angiography,86 cases of patients diagnosed with acute coronary comprehensive disease were recruited for the study and randomly divided into two group,control group(n=46):On the day of admission to the load dose aspirin 300mg and clopidogrel 300mg,maintenance dose:clopidogrel 75mg/d and aspirin 100mg/d.Experience group(n=40):on the day of admission to the load dose aspirin 300mg and ticagrelor 180mg,maintenance dose:ticagrelor 90mg bid and aspirin 100mg/d.After one week,the inhibition rate of platelet was detected by thromboelastography(TEG)and test routine blood test,Platelet count,the inhibition rate and platelet resistance was compared between the two groups.Results With no difference in the case of general information,In safety,Platelet count in control group(228.35±25.45)×10~9/L vs experience group(235.87±36.74)×10~9/L,(t=1.087,P>0.05).In platelet inhibition,the inhibition rate of control group(74.5±9.8)vs experience group(85.3±10.5),(t=4.931,P<0.01).There were 9 patients(19.57%)have clopidogrel resistance in control group,and 1patient(2.5%)has ticagrelor resistance in experience group(?~2=4.517,P=0.034).Conclusion Ticagrelor has stronger inhibition of platelet aggregation function and lower platelet resistance than clopidogrel,the antiplatelet effect of ticagrelor is better than that of clopidogrel.
