Objective:Micro-inflammation persists in patients with peritoneal dialysis.Many cytokines mediate the development of micro-inflammation.The age of dialysis affects the effectiveness of dialysis.Peritoneal dialysis is often failed due to the peritoneal dialysis-related peritonitis.The relationship between the expression level of cytokines and peritoneal dialysis-associated peritonitis and dialysis age is currently less studied.This study hypothesizes its association and conduct relevant research.Methods:Clinical cases were gathered 30 patients from March 2016 to August 2017,Nephrology Department in the Guangzhou General Hospital of Guangzhou Military Command who with Continuous ambulatory peritoneal dialysis.There were 12 cases of peritonitis(positive group),according to the diagnostic criteria of peritoneal dialysis related peritonitis,(The positive group was compared before and after treatment,which was pre-treatment group and post-treatment group respectively).The remaining 18 cases were non-peritonitis(negative group)。Thenegative group,according to the age of dialysis,12 months,12-36 months,and more than 36 months,was divided into three groups,named,A,B and C,with 6 people in each group.Collected clinical demographic data of selected patients and collected overnight fasting peripheral venous blood to detect hemoglobin,serum albumin,serum potassium,and serum uric acid.Take the overnight Peritoneal dislysis solution samples and detectde cytokines,related to inflammation,VEGF、PDGF-BB、IFN-γ、TGF-β1、IL-6、IL-10、TNF-a、IL-7、IL-13、IL-11、ICAM-1 used the QAH-CUST-12 protein chip and performed comparative analysis.Result:1.In the positive group,the treatment group and the negative group comparison.the positive group factor IL-6,VEGF,and serum uric acid were highly expressed in the negative group and were down-regulated after treatment(P<0.05),and there was no difference between the treated group and the negative group;The levels of hemoglobin and serum albumin were lower than those in the negative group and significantly increased after treatment(P<0.05).After treatment,albumin was still lower than the negative group(P<0.05).2.Among the three groups A,B and C,PDGF-BB and TNF-a were found to be the highest in group A,followed by group B,and lowest in group C.The difference was statistically significant.(P<0.05).3.Correlation analysis revealed that hemoglobin was negatively correlatedwith cytokines VEGF,IL-6,IL-10,ICAM-1;serum albumin and factors VEGF,IL-6,IL-10,ICAM-1,IL-13,IL-11 was negatively correlated;serum uric acid was positively correlated with factors IL-6,IL-10,IL-13,IL-11 and VEGF.There was a positive correlation between IL-6 and ICAM-1 and IFN-γ.VEGF was positively correlated with IL-6,IL-10,IL-11 and IL-13.Conclusion:When peritoneal dialysis complicated with peritonitis,cytokines IL-6 and VEGF were highly expressed in dialysate and decreased with the improvement of therapy.It is suggested that IL-6 and VEGF play a key role in maintaining and exacerbating peritonitis and can be used as inflammatory markers of peritonitis.With the prolongation of dialysis time,PDGF-BB and TNF-a in the dialysate gradually decreased,it is guessed that it may play a major role in the early stages of peritoneal dislysis.The expression level of cytokines was closely related to anemia,nutritional status and serum uric acid,and there was a clear correlation between cytokines.
