To Observe The Clinical Efficacy Of Modified Qiming Granule To Intervene Eye And Kidney Damage Of Diabetic Microangiopathy Based On The Idea Of Combining Disease And Syndrome

Objective:Guided by "combination of disease and syndrome",From the perspective of diabetic microvascular lesion eye kidney treatment together to observe the clinical efficacy of Qiming granule about Diabetic microvascular eye and renal damage.To explore two disease complicated by diabetes microvascular lesions of TCM intervention to prevent and control the basis.Methods:Using randomized,controlled clinical trials to nalysis the 70 cases of Diabetic microvascular lesions in the eye with renal disease that conforms to the Qi and Yin deficiency,vein blood stasis resistance type.The 36 cases of disease and syndrome differentiation were given Qiming particles plus rhubarb,heterophylla and other TCM,Fixed drugs group of 34 cases were given jiust Qiming particles,12 weeks for a course of treatmen.To observe albumin to creatinine ratio,Glomerular Filtration Rate,Liver and kidney function,Glycated hemoglobin,the vision,fundus(Ophthalmoscope,fundus color photography)And TCM symptoms and other indicators.To evaluate the clinical efficacy and safety of Qiming granule in treating di abetic microvascular lesionst that with Diabetic retinopathy and diabetic nephropathy.Results:1.the fixed drugs set off in 2 cases,Disease dialectical group of urine albumin creatinine ratio than fixed drugs reduce significantly,The difference was statistically significant(P<0.01).the group of disease and syndrome differentiation was statistically significant on creatinine an d GFR(P<0.01),not only Within the group,but also between tgrops.2.Comparing Vision and fundus within groups(P<0.05),two groups all can improve vision and fundus,There was no significant difference between the two groups in visual acuity and fundus P>0.05).the group of disease and syndrome differentiation has statistically significant within group about Glycosylated hemoglobin were compared(P<0.05).There was no signific ant difference between the two groups(P>0.05).3.According to TCM score table,the disease dialectical group:13 cases were markedly effective,effective 19 cases,4 cases were ineffective,the total effective rate was 88.89%.the fixed drugs group:7cases were markedly effective,effective 15 cases,10 cases were ineffective,the total effective rate was 68.75%.Comparison between the two groups(P<0.05),the difference has statistical significance.4.ACR with patient’s age,duration of DM,duration of DR,fundus,creatinine,Merge hyperten sion glycosylated hemoglobin,Combined with hypertension were positively correlated(P<0.05),and negatively correlated with GFR(P<0.05).Conclusion:1.Qiming granule additives treatment of qi and yin deficiency,pulse blood stasis type diabetic eye and nephropathy curative effect is better.2.Qiming granule additives can reduce the qi and yin deficiency,pulse stasis type diabetic eye damage in patients with urine microalbumin creatinine ratio,serum creatinine,And can simultaneously improve glomerular filtration rate,vision,fundus,3.Qiming granule additives can significantly improve TCM syndrome.4.Urinary microalbumin creatinine ratio and the patient’s age,duration of DM,DR course,fundus,Ccr,Hb A1 c,and history of hypertension were positively correlated(P<0.05),and negatively correlated with GFR(P<0.05).
