Objective1.To investigate the clinical features of colorectal polyposis and reasonable endoscopic screening to reduce the mortality of colon cancer.2.To investigate the curative effect of endoscopic and surgical treatment of colorectal carcinogenesis,including hospitalization time,hospitalization costs,postoperative fasting time,complications,and follow-up after polypectomy,in order to regulate the treatment of early colorectal cancer for reference.Methods1.To collect the affiliated hospital of Qingdao university endoscopy room on January1,2005 to December 31,2016,colonoscopy findings and pathologic diagnosis of adenomatous polyp with severe dysplasia or cancer of the clinical data of 131 patients,including(age,gender,body mass index(BMI),smoking history,clinical symptoms,with or without anemia(excluding blood system diseases,gastrointestinal bleeding,etc.),the digestive system tumor markers(CEA,CA199and AFP),defecate occult blood,precancerous polyps,diameter size,organization form and pathological type).2.The medical records of all the patients with colorectal polyposis collected above were divided into 2005-2010 and 2011-2016 in the two groups,52 cases and 79 cases respectively,comparedtwogroupsofage,gender,clinical symptoms,pathogenesis,pathology type and treatment differences.3.Among 131 patients with colorectal polyposis,80 underwent endoscopic resection and 51 underwent conventional surgical resection,the diameter,location,shape,and pathology of cancerous polyps were also analyzed.The hospitalization time and hospitalization costs of the two treatments were analyzed.At the same time,analysis of the two methods of treatment,postoperative fasting time,length of hospital stay,hospital expenses complications and follow-up.Results1.The clinical data of colorectal polyp cancer patients.A total of 131 patients with colorectal polyposis were detected from 2005 to 2016,ranging from 29 to 81 years of age,with a median age of 60 years and a sex ratio of2.12:1.The patient’s BMI was mostly concentrated at 23.0-29.9 kg/m~2.The main clinical features is bloody.Most of the lesions located in the distal colon(including the rectum,sigmoid colon,descending colon,and spleen)accounted for 83.2%.The colorectal cancerous lesions(≥3cm)were 43.6%.In terms of morphology,53 cases of non-pedunculated colon polyps is accounting for 40.5%and 78 cases of pedunculated polyps is accounting for 59.5%.In terms of pathological types,63 cases of severe dysplasia is accounted for 48.1%and 68 cases of intramucosal cancer is accounted for51.9%.2.The clinical features of colorectal polyp canceration change analysis.Compared before and after 6 years and polyp canceration increased earlier detection and average age rose from 59 to 61 years old,cancer patients have the tendency of increase with the age.No statistical significance before and after 6 years and sex ratio(c~2=3.02,P>3.02).Before and after 6 years cancerous parts no statistical significance(P=0.70).3.The canceration of colorectal polyp endoscopic and surgical curative effectanalysis and follow-up.Smaller polyp canceration(2cm or less),pedicle and precancerous polyps choose endoscopic resection,more with strong and wide(3cm)or higher,non-pedunculated,cancerous person chooses more surgical resection,compared two groups was statistically difference.Multiple factors logistic analysis,hospitalization expenses,fasting time is endoscopic treatment is surgical treatment has obvious difference.Postoperative review and follow-up the results found that the endoscopic group had faster wound healing,shorter recovery time,and less scars than the surgical group.Conclusion1.As the age of patients gradually increased,the detection rate of colorectal polyp cancer was the most important among the elderly.2.Patients aged over 40,obese,with smoking history,clinical manifestations of hematopoietic,fecal occult blood,and colorectal polyps are susceptible to cancer,and screening of electronic colonoscopy should be conducted on a regular basis.3.Endoscopic treatment than traditional surgery to remove cancerous colorectal polyp has small trauma,short recovery time,low cost,is worth popularizing in clinic.4.According to the results of follow-up prompt endoscopic treatment is surgical postoperative healing fast,small scar.
