Objective: By conduct a meta-analysis to analysis the incidence of postoperative pancreatic fistula、 complications and mortality between two types of pancreaticojejunostomy after pancreatoduodenectomy, evaluate effect of these two methods. Methods: Computer retrieve all of the clinical trails which refer to these two types of pancreaticojejunostomy after pancreatoduodenectomy from the PubMed、Cochrane Library、EMbase since these databases was established to March 2018.Revman 5.3 software was used to analyze the incidence of postoperative pancreatic fistula, clinical relevant pancreatic fistula, postoperative complications, serious complications, postoperative mortality, reoperation rate, postoperative hospital stay between these two types of pancreaticojejunostomy. Results: Nine articles including 1219 patients meet inclusion criteria.Meta-analysis showed that invagination pancreaticojejunostomy was more advantageous in reducing the incidence of clinical relevant pancreatic leakage and serious complications after operation, but there was no significant statistical difference. While considering other parameters such as postoperative pancreatic fistula, postoperative complications, postoperative mortality, reoperation rate, postoperative hospital stay, there was no difference found between these two motheds. Conclusion: According to the existing evidence,we can not define which is the better.
