Clinical Report Of 38 Cases Of Chylous Leakage After Abdominal Surgery

Objective: By comparing and analyzing three conservative treatments of CF after abdominal surgery,we can find the best treatment strategy and improve the curative effect.Methods: A retrospective study was conducted on 38 patients with CF after abdominal surgery from September 2007 to September 2017 at Second Hospital affiliated of Chongqing Medical University.Of these,4cases were discharged with tubes,resulting in incomplete clinical data,no results of telephone follow-up,while 4 cases were treated differently,excluding 8 cases,and the remaining 30 cases were included in the study.Finally,7 cases in LFD group,10 cases in TPN group,and 13 cases in TPN+S group,all were cured by conservative therapy.General clinical data such as pretreatment sex,age,weight,first-day lead flow,NNIS rating were analyzed and compared(P>0.05).The clinical efficacy of the three treatments was compared with that of CF after treatment,normal diet,drainage and hospitalization.Metrological data were expressed by mean ± standard deviation(x±S)and T test was selected for inter-group comparison.Counting data is expressed as a percentage(%)and compared using the square 2 test.Results: After conservative treatment,the duration of CF disappearance,normal eating time,drainage time and hospitalization time of LFD group were respectively(24.29±5.59)d,(26.43±4.93)d,(29.00±4.62)d,(32.57±4.86)d;TPN(17.10+1.20)d,(20.20±1.03)d,(23.30±1.64)d,(26.60±2.17)d;TPN+S(15.69±1.03)d,(18.77±1.01)d,(21.54±1.45)d,(24.38±1.56)d;The differences between the three groups were statistically significant(P<0.05).Conclusion: Total parenteral nutrition+somatostatin should be the first choice for CF after abdominal surgery,its curative effect is accurate and significantly superior to other conservative treatments.It is worth popularizing in clinic;A low-fat diet can be recommended as a post-cure consolidation measure,rather than as a preferred treatment.
