Diagnostic Value Of ¹⁸F-FDG PET/CT In Patients With Fever Of Unknown In Infectious Department

Objective:To investigate the diagnostic value of ¹⁸F-FDGPET/CT in patients with FUO,the timing of clinical 18F-FDG PET/CT examination,and the correlation between positive serum biochemical indexes and 18F-FDG PET/CT results.Methods:Consecutive patients who visited the First Affiliated Hospital of China Medical University with FUO referred for ¹⁸F-FDGPET/CT in 2012 July to 2011March were included retrospectively.A retrospective analysis of¹⁸F-FDG PET/CT was performed using with fever of unknown origin.The analysis of demographic characteristics（including gender,age）,fever course,serum biochemical（including white blood cell count,neutrophils,lymphocytes,C-reactive protein,erythrocyte sedimentation rate,procalcitonin,ferritin）and physical and other related test results were also included in our study.SPSS 20.0 software was used for data processing,including two sample withindependent sample t test.Results:66 cases were made a clear diagnosisby the ¹⁸F-FDG PET/CT,including 31cases of infectious diseases（34.8%）,15 of malignancies（16.9%）,20 of noninfectious inflammatory diseases（22.5%）.As for the remaining 23 cases（25.8%）,clear diagnosis still cannot be made after 3-6 months of follow-up.The sensitivity,specificity and accuracy were 84.5%,25.8%and 64.0%,respectively.The diagnostic accuracy of¹⁸F-FDG PET/CT in malignancies,infectious diseases and NIIDs were 100%,61.3%and 75%.Positive results were obtained for lymphnode biopsy in 3 patients with abnormal lymph node FDG.Compared with the serum biochemical indexes of effective group and ineffective group,only the C-reaction protein（CRP）had significant difference between the two groups.C-reactionprotein was 100.50±65.2mg/L in valid group and 66.64±60.92mg/L in invalid group（P<0.05）.There was no significant difference in fever course between valid and invalid groups（P>0.05）.Conclusion:The sensitivity and diagnostic accuracy of ¹⁸F-FDG PET/CT in the patients with FUO are very high,and the level of CRP is a good indicator for improving the accuracy of¹⁸F-FDG PET/CT.On the other hand,¹⁸F-FDG PET/CT can provide accurate localition of the lesion biopsy.
