The Association Between DBP Rs4588 Gene Polymorphism,Microembolic Signal And Prognosis In Large-artery Atherosclerosis Stroke Patients

Objective: To investigate the relationship between Vitamin D Binding Protein(DBP)gene polymorphisms and ischemic stroke of pations with large-artery atherosclerosis,as well as the microembolic signals in the ipsilateral stenotic artery and prognosis.Methods: We studied 203 patients diagnosed with large artery atherosclerostic(LAA)stroke and 121 controls.The LAA stroke patients were divided into clinical subgroups according to the presence of microembolic signals and the modified Rankin Scale(m RS)at 3 months after stroke.Collected the general situation of the patients and their previous history.Venous blood was collected under fasting condition.The DBP rs4588 gene polymorphism was examined by polymerase chain reaction and restriction fragment length polymorphism.The microembolic s ignals(MES)were monitored by transcranial doppler(TCD)for 60 min within 72 h of stroke onset.Prognosis was assessed by the modified Rankin Scale(m RS)at 3 months after stroke.The data were analyzed by SPSS 20.0 software.Results: 1.The case group had a significant higher rate associated with hypertension、smoking、alcoholism、CRP and GLU(p<0.05).The case group had a significant lower rate associated with HLD.The AA+CA genotypes and allele A frequencies of the rs4588 gene polymorphism were significantly higher in the LAA group than in the control group(71.43% vs.44.63%,P=0.032;41.38% vs.24.38%,P<0.001).After adjusting for other risk factors,the differences remained significant(OR=3.468,95%CI:1.971-6.101)).2.The MES+ group had a significant lower rate associated with CRP(p<0.05).The AA+CA genotypes and allele A frequencies of the rs4588 gene polymorphism were significantly higher in MES + patients compared to MES-patients(84% vs.67.32%,P=0.018,53% vs.37.58%,p<0.01)。After adjusting for other risk factors,the differences remained significant(OR=2.770,95%CI:1.195-6.421).3.The poor prognosis group had a significant higher rate associated with TG and GLU(p<0.05).There were no significant differences between t he AA+CA genotypes and allele A frequencies of rs4588 and m RS Score(81.0% vs.64.0%,P =0.1;54.17%vs.45.83%,P=0.509).Conclusion: DBP rs4588 gene locus is polymorphic in the population of Qingdao,Shandong province.Our findings suggest that the DBP rs4588 gene polymorphism might be related to the occurrence of LAA ischemic stroke and microembolic signals,while there was no associated with the prognosis of LAA type cerebral infarction.
