Intervene In The Self-management Scale Of Patients After Mechanical Heart Valve Replacement

ObjectiveTo investigate the current situation of self-management in patients after mechanical heart valve replacement(MHVR)and the influential factors,adopt Self-Management Scale For Patients After Mechanical Heart Valve Replacement intervene the patients after surgery.Evaluate that scale’s clinical application effect,use for reference in self-management education.Methods :1.Investigate the influential Factors of Self-Management in Patients after Mechanical Heart Valve Replacement.The convenience sampling method was used to obtain the sample in Chongqing Municipality,and 400 participants after MHVR at a month in3 Grade Ⅲ A hospitals were recruited.Use Self-Management Scale For Patients After Mechanical Heart Valve Replacement and the questionnaire of basic demography data edited by ourselves collect the data in face-to-face or telephone interview.Use multi-classification logistic regression to confirm the influential factors.2.Intervene effect for patients in Self-Management Scale for patients after Mechanical Heart Valve Replacement.The convenience sampling method was used to obtain the sample in Chongqing Municipality,and 365 participants after MHVR will leave hospital immediately in 2 Grade Ⅲ A hospitals were recruited.Use Self-Management Scale for patients after Mechanical Heart Valve Replacement and Short-Form Quality of Life Questionnaire-12,(SF-12)collect the data in face-to-face interview,then guide the patients in experimental group with their weak links,and adopt conventional educational channel for patients in control group,reevaluate them in one month after discharge,compare the scores before and after.Results:1.The self-management level of patients after MHVR and influential factors.The score of patients between 79 and 119.The average score of patients was 96.81±7.86.,scoring average is 71.7%.Among these patients,there are 64 patients with higher scores,between 105~119(16%),260 patients with medium scores,between 89~104(65%),and 76 patients with lower scores,between 76~88(19%).Compared every dimension’s scoring rate,the highest scoring rate is NO.2 dimension,96.7%,the lowest scoring rate is NO.5 dimension,42.3%,next is NO.1 dimension,54.3%,The influential factors.sex,age,education level and the resident manner.2.The intervene application in Self-Management Scale for patients after Mechanical Heart Valve Replacement.There have 365 patients in this research,196 in intervene group and169 in control group.The score of Self-Management Scale for patients after Mechanical Heart Valve Replacement before discharge patients in experimental group vs control group is 90.92±7.81 vs 91.51±6.92,the score of Self-Management Scale for patients after Mechanical Heart Valve Replacement after discharge one month patients in experimental group vs control group is 102.66±7.05 vs 92.22±8.27,The score in two groups’ patients before discharge of Self-Management Scale for patients after Mechanical Heart Valve Replacement difference was not statistically significant,but there was statistically significant in two groups’ patients after discharge one month,but but there was not statistically significant in self-efficacy and emotion management dimention.The score of SF-12 before discharge patients in experimental group vs control group is 493.33±32.06 vs 497.27±28.82,after discharge one month,the score of SF-12 in experimental group vs control group is520.68±28.73 vs 502.83±30.35.The score in two groups’ patients before discharge of SF-12 difference was not statistically significant,the scores’ difference was statistically significant after discharge on month.But in Physical Functioning,Role Physical,Role Emotional,Mental Healthdimension,their difference was not statistically significant.ConclusionThe self-management level of patients after MHVR need to improve,especially in symptom management and information management dimension,.Gender,age,resident manner,standard of culture are influential factors to self-management ability.The male,advanced age,live with distant relatives or housemaid,low education level’s patients’ self-management ability are low,the medical workers should strengthen health education in these patients and their relations.Adopt Self-Management Scale for patients after Mechanical Heart Valve Replacement educate patients after Mechanical Heart Valve Replacement will enhance patients’ self-management ability outstanding,improve them recovery.
