Object:To evaluate the results of micro hollow screws in the treatment of distal phalanx fractures of the hand and compare the results of various operative methods to improve the clinical outcome of distal phalanx fractures.Methods:The clinical materials of 78 distal phalanx fractures were collected from the 2015-2018 at the Third Hospital of Hebei Medical University.Among them,53 males and 25 females,aged 18-56 years,with an average age of 37.Causes of injury:sharp-injury in 11 cases,bruise in 32cases,stabbing in 10 cases,crush injury in 25 cases;open injury in 57 cases,closed injury in 21 cases;fracture types:43 cases of transverse fractures,32cases of oblique fractures,comminuted fractures in 5 cases;35 cases were fixed with micro hollow screws,and another 43 cases were fixed with K-wires.All 78 cases were followed up.The mean follow-up time was 3 months(range1 to 6 months).The results were compared regarding to pain,finger deformity,fracture union rate,and joint mobility,etc,TAM method was used to evaluate clinical curative effect.Results:There was no significant statistical difference between the two groups before operation(P>0.05).The results of the 35 cases of micro hollow screw fixation were better than the 43 patients with K-wires fixation in the aspects of finger appearance and TAM function evaluation,and the difference was statistically significant.(?~2=8.175,P<0.05);?~2=9.025,P<0.05;?~2=8.711,P<0.05;?~2=8.418,P<0.05).The results of the 35 cases of micro hollow screw fixation were better than the 43 patients with K-wires fixation in the aspects of clinical union rate and pain evaluation,and the difference was statistically significant.(?~2=2.871,P>0.05;?~2=1.37,P>0.05).Conclusions:1.The micro hollow screw fixation of distal phalanx fractures can be used to avoid complications and shorten recovery period.2.The design and conduction of the study was strictly followed to randomized controlled principle,and the clinical outcome gained from this study can be used to guide for the treatment of such type of injury.3.The major risk factors for this injury is high-energy trauma.4.The different types of distal phalanx fractures should be treated individually to improve the clinical outcomes.
