The Value Of Prenatal Ultrasonic Score For Placenta Invasion With Pernicious Placenta Previa.

Objective : We sought to evaluate the significance of ultrasound parameters for placenta invasion of pernicious placenta previa and formulate a standardized evaluation.Methods: This is a retrospective review,Summary of patients with≥1prior cesarean who received an ultrasound diagnosis of placenta previa in the third trimester and successfully delivered their boby at shengjing hospital during 2015-2017.The images of ultrasound include anterior placentation,loss of retroplacental clear zone,presence of lacunar spaces,smallest of myometrial thickness and blood of uterine-bladder interface.Diagnosis of placenta invasion is based on pathology.According to the pathology.Results: 173 gravidas met criteria,of which 61 gravidas were diagnosed invasion confirmed on pathology.Single factor analysis show that there were no difference in age,abortion,and prior caesarean.Anterior placentation,loss of retroplacental clear zone,presence of lacunar spaces,smallest of myometrial thickness and blood of uterine-bladder interface were significantly with placenta invasion(p<0.01).Constructing ROC,yielded an area under the curve of 0.921(95%CI:0.889-0.961).Using logistic egression,generating a predictive equation,assigning a weighted value to each ultrasound parameter.presence of grade 3 lacunar was weight most,so impart 4 scores.According to scores,constructing ROC,yielded an area under the curve of 0.970(95%CI:0.946-0.994).Conclusion: This research privides prediction of each ultrasound parameter in diagnosis of placenta invasion.It will be helpful in predicting the risk of placenta invasion.Early detection for invasion,making adequate preparations,it has an important guiding significance in reducing the rate of hysterectomy and postpartum hemorrhage.
