Objective: To assess the early changes of myocardial contractile function in heart failure patients with preserved ejection fraction(HFPEF)by three-dimensional global area strain(GAS).Methods: Totally 30 patients with HFPEF and 30 healthy volunteers underwent two-dimensional echocardiography,tissue Doppler imaging and three-dimensional speckle tracking echocardiography,parameters including left atrial diameter at end-systole(LAd),left ventricular diameter at end-diastole(LVEDd),interventricular septum thickness at end-diastole(IVSTd),left ventricular posterior wall thickness at end-diastole(LVPWTd),peak E(E),peak A(A),the ratio of E/A,e ′ at interventricular septal and left ventricular lateral wall(e ′),the ratio of E/e ′,left entricular volume at end-diastole(LVEDV),left ventricular volume at end-systole(LVESV),left ventricular ejection fraction(LVEF),left ventricular global area strain(GAS)were calculated and compared.The correlations of GAS with LVEF were analyzed.Results: There was no differences of LVEDd,E,LVEDV,LVESV,SV and LVEF between the two groups,(P>0.05).LAd,IVSTd,LVPWTd was significantly higher in HFPEF group than those in control,left ventricular diastolic function was reduced in HFPEF group with lower E/A,e′and higher A,E/e′;Compared with the control,GAS was significantly reduced in HFPEF group([-0.1898 ± 0.0836]vs[-0.2522 ±0.0473]),both P<0.05.There was a negative correlation between GAS and LVEF(r=-0.50,P<0.05).Conclusion: The left ventricular myocardial contractile function has reduced before LVEF changed,which can be detected by 3D-STE with GAS.
