Objective : To investigate the incidence of orthostatic hypotension(OH)in elderly patients with multimorbidity and polypharmacy,and to explore the relationship between mild anemia and orthostatic hypotension in this population.Methods: 175patients(mean age 73.79±9.53 years,88 women)with multimorbidity and polypharmacy were divided into 60-79 years old group(n= 116)and ≥ 80 years old group(n=59).Their supine and orthostatic blood pressures were measured.According to their blood pressure levels they were divided into orthostatic hypotension(OH)group and non-orthostatic hypotension(NOH)group.Assessments included the blood biochemical parameters and general clinical data.Results: Among 175 patients,36(20.57%)cases with OH were found.The incidence of OH and mild anemia were significantly higher in 60-79 years old group than in ≥ 80 years old group(p<0.05).The incidence of mild anemia was higher in OH group than NOH group(p<0.05).On Logistic regression analysis,OH appeared significantly correlated to mild anemia(OR = 5.261,95%CI: 2.231-12.406,p=0.000),even after controlling for factors of age,BMI,albumin,pre albumin,total cholesterol,high density lipoprotein cholesterol,and cystatin C,which may influence the indices of anemia(OR = 4.401,95%CI: 1.529-12.667,p=0.006).Conclusions: OH and mild anemia status are closely related to age,which may occur more frequently in the advanced aged patients.Mild anemia status is an independent risk factor for OH in elderly patients with multimorbidity and polypharmacy,which may provide reference for risk prediction and treatment of OH.Correction of anemia is of certain value for intervention management of OH in this population. |
