Objective: To observe the application of Lacrosse NSE in the percutaneous coronary intervention to evaluate the safety and efficacy on Lacrosse NSE angioplasty for treating the patients with limited and calcified coronary lesions.Methods: A total of 86 consecutive patients with limited and mild or moderate calcified lesion confirmed by coronary angiography and IVUS who underwent PCI in Tangshan Gongren Hospital from May 2016 to May 2017 were studied.The patients were randomly divided into two groups: the Lacrosse NSE group(n=44)and the Ordinary balloon group(n=42).Three cases in the Ordinary Balloon group did not satisfactorily achieve dilatation and were transferred into the Lacrosse NSE group.Analysis general information,Angiographic images,Balloon slip frequency,IVUS parameters before and after PCI,effect of stent placement,the total duration of balloon inflation,Intervention complications,stent thrombosis during hospitalization,Left ventricular ejection fraction,stent restenosis and the MACE for in-hospital,1 month,3month and 6month after PCI were recorded in all patients.All the data was statistically analyzed with SPSS 17.0 software package,and the difference was statistically significant with P<0.05.Results: 1 Between the Lacrosse NSE group and the ordinary balloon group in clinical features include gender,age,body mass index,smoking,history of hypertension,diabetes,hyperlipemia,etc,and the difference were no statistically significant(P>0.05).2 Before stent implantation,calcification radian,calcification length ratio,reference cavity area,minimum lumen CSA,Stenosis rate of lumen,Plaque burden and Plaque area in the Lacrosse NSE group and the ordinary balloon group,the difference were no statistically significant(P>0.05).3 After stent implantation,the final minimum stent CSA(6.30±0.67mm2 vs 5.33±0.71mm2),reference lumen area(8.15±0.54 vs 7.68±0.59)mm2,acute lumen gain(3.22±0.54mm2 vs 3.51±0.47mm2)were significantly larger in the Lacrosse NSE group than that of the ordinary balloon group,and the difference were statistically significant(P<0.05).The balloon slip frequency(5.26±1.33 time vs 3.70±1.25time),the operation time(89.97 ± 9.12 min vs 81.84 ± 7.51min)were significantly larger in the ordinary balloon group than that of the Lacrosse NSE group,and the difference were statistically significant(P<0.05).4 Stents symmetry,stick a wall,stent expansion rate,blood loss,blood vessels in the Lacrosse NSE group and the ordinary balloon group,the difference were no statistically significant(P>0.05).5 Left ventricular ejection fraction、Stent restenosis and Major adverse cardiovascular events during hospitalization,1month,3month and 6 month in the Lacrosse NSE group and the ordinary balloon group,the difference were no statistically significant(P>0.05).Conclusion: The Lacrosse NSE used in coronary artery mild or moderate calcified lesion can effectively increase the acute lumen gain.It can improve the effect of stent implantation,without increasing in the incidence of postoperative MACE events. |
