Effects Of Inhalation Sucrose On Cough Reflex

Objectives:A prospective cohort study was conducted to investigate the antitussive effect and the mechanism of cough relieving of sucrose whether it can be used in patients with chronic cough through capsaicin cough provocation test and sucrose atomization.To relieve cough symptoms,seek new antitussive drugs and provide new ideas and clinical treatments for the treatment of chronic cough.Methods:The subjects of this study were 60 patients with chronic cough admitted to our department from August 2016 to December 2017.Inclusion criteria:the only symptoms or main symptoms were cough,duration of cough was more than 8 weeks,and chest X-ray was nortmal.Exclusion criteria:patients with cough after respiratory infection in the past 3 months,patients with mental illness,and patients with poor medical compliance.In this study,a prospective cohort study was used to randomly divide 60 patients into two groups.Two groups of patients were tested for cough threshold with the capsaicin cough challenge test and cough severity with the VAS score.The treatment of sucrose atomization was used,and the other group was treated with codeine.After 3 days of treatment,capsaicin cough threshold and VAS score were retested.100 local healthy people were selected,50 were used as the standard value of the capsaicin cough threshold,and the normal person’s cough threshold was measured.Another 50 people were first treated with sucrose atomization and then the capsaicin cough threshold was measured.The data obtained in this study were statistically analyzed by SPSS20.The measurement data of normal distribution used t-test.The measurement data of non-normal distribution used the Wilcoxon signed rank test.Count data used Chi-square test.P was less than 0.05 and the difference was considered statistically significant.Result:The cough threshold value of the patients with chronic cough after the treatment of sucrose atomization is significantly higher than before the treatment by the capsaicin cough provocation test.The threshold value of the patients with chronic cough after the treatment of the codeine is significantly higher than before the treatment.There were no significant difference between after the treatment of sucrose atomization and the codeine,but the median,minimum and maximum values of the capsaicin cough threshold were the same.P75 values of the treatment of codeine group is higher than that of sucrose atomization group.The VAS score of the patients with chronic cough after the treatment of sucrose atomization is significantly lower than before the treatment.The VAS score of the patients with chronic cough after the treatment of the codeine is significantly lower than before the treatment.The VAS score of the patients with chronic cough after the treatment of the codeine is significantly lower than after the treatment of sucrose atomization.The median,quartile,minimum and maximum of VAS score after the treatment of the codeine is lower than after the treatment of sucrose atomization.Healthy people group:The median of cough threshold value is about 250umol/L.The cough threshold value of the healthy people group after the treatment of sucrose atomization is significantly higher than before the treatment by the capsaicin cough provocation test.Conclusions:Sucrose by inhibiting peripheral cough receptors to improve the cough threshold and have antitussive effect.The cough threshold is increased after sucrose atomization in patients with chronic cough,so sucrose can be used for the treatment of chronic cough.Sucrose atomization is a kind of treatment of cough and can be used for clinical treatment;Capsaicin cough provocation test is safe,the incidence of adverse reactions is low and easy to operate.It is widely used easy in clinic.VAS score can evaluate the symptoms and degree of cough in patients comprehensively.The operation is simple and convenient for clinical use.The combination of capsaicin cough provocation test and VAS score can well evaluate the cough condition of patients;The cough threshold of healthy people is above 125umol/L by the capsaicin cough provocation test.Different eating habits,cough threshold is different.
