Objective1.To describe the three distribution of Brucellosis from 2012 to 2017 in Hunan province and to research the current situation and development trend of Brucellosis,as well as to provide theory evidences for measure making and works of Brucellosis prevention and control.2.To research the KAP situation of Brucellosis of sheepman in Hunan province,to analyze the factors influencing the knowledge and behavior of brucellosis,to explore the correlation between knowledge awareness and behavioral habits of sheepman,as well as to provide basis for the intervention of education.Methods1.According to Hunan disease report information system,to analyze the three distribution featrues and diagnosis of Brucellosis from 2012 to 2017 in Hunan province,thus to reveal high risk time,high risk areas,high risk groups.2.A questionnaire is used to survey the sheepman in Hunan province by using stratified random sampling method.The contents of the survey include general demographic characteristics,knowledge of disease prevention and related behaviors,the data are analyzed by SPSS21.0 statistical software.Results1.A total of 558 cases of Brucellosis were reported in Hunan province from 2012 to 2017,the average annual incidence was 0.13/100000 with an average annual increase of 56.93%,the difference of incidence in different years was statistically significant(?2=261.74,P <0.01).There are case distribution in 14 prefectures and cities in Hunan province,among which the five cities with the highest incidence are Chenzhou,Huaihua city,Changsha city,Yongzhou city and Shaoyang city,accounting for 68.99%(385/558).Brucellosis can occur at any time,the disease occurs at least one peak every year,mainly in summer and autumn,and it is the peak period in April and July,accounting for 51.61%(288/558).There was a difference in the proportion of the peak periods in each year,and it was statistically significant(?2=12.66,P<0.05).The majority of cases were male,accounting for 75.27%(420/558),there were more male cases than female cases in all years,but the difference was not statistically significant(?2=2.924,P>0.05).Brucellosis can occur at any age,the minimum age of onset is 1 year,the most is 84 years old,and the cases are mostly between 30 and 70 years old,accounting for 85.48%(477/558).The occupation is mainly farmers,accounting for 62.54%(349/558),and followed by the herdsmen,accounting for 4.48%(25/558).Case classification is mainly laboratory confirmed cases,accounting for 87.99%(491/558).The case report was mainly from clinical hospitals,accounting for 77.96%(435/558),followed by the CDC,accounting for 20.61%(115/558).2.Only 98 people have heard of brucellosis among 331 sheepmen from Hunan.The total awareness rate of the knowledge of brucellosis prevention for 98 people was 44.35%.Single factor analysis shows that age,educational level and regional category are statistically significant(?2=12.330,P=0.016;?2=21.813,P<0.001;?2=30.838,P<0.001).Multivariate logistic regression analysis shows that educational level and regional category are statistically significant,the prevention knowledge in middle school and high school or above is higher than primary school(RO???=5.541,95%CI:2.424~12.665;RO?????=6.255,95%CI:2.438~16.045),the prevention knowledge in middle and low incidence areas is lower than high incidence areas(RO?????=0.141,95%CI:0.057~0.351;RO?????=0.094,95%CI:0.036~0.245).Sheepmen have less than 50%protection against all kinds of contact with sheep,and protective rate difference had statistical significance between eligible person and Ineligible one(?2=51.449,P=0.000;?2=44.985,P=0.000;?2=44.794,P=0.000;?2=39.383,P=0.000).There are six high-risk behaviors for sheepmen that may be associated with brucellosis,including not wearing latex gloves when contacting animals or products,not washing hands when contacting sheep,sheep placenta or products,eating raw or not cooked meat,drinking raw milk,processing and eating sheep placenta,selling sheep placenta.The ratio of them is respectively78.96%,27.27%,1.52%,1.22%,9.97%,5.45%,the rate difference had statistical significance between eligible person and Ineligible one about the question of not wearing latex gloves when touching livestock or products and not washing hands after contact with sheep,placenta or product(?2=37.475,P=0.000;?2=13.750,P=0.000).There is a weak positive correlation between the score of the prevention knowledge of brucellosis and their good behavior.Conclusion1.The cases of Brucellosis from 2012 to 2017 in Hunan province showed a significant upward trend,with wide geographical distribution and obvious seasonal distribution.The occupation is dominated by farmers and herdsmen,and the main infectious population of brucellosis is the middle-aged and middle-aged workers.2.The awareness rate of the prevention knowledge of brucellosis among sheepmen in Hunan province is low,and there are high-risk behaviors in life and work.So We need a healthy education for brucellosis.If sheepmen can master more knowledge about brucellosis,it may be beneficial to develop good behavior habits.
