| Objective: to investigate the perfusion characteristics of arachnoid cyst in children with arachnoid cyst by arterial spin labeling.Materials and methods:20 arachnoid cyst patients with unilateral lateral fissure who were admitted to our hospital from January 2017 to March 2018 were selected as the case group.All of them were male,aged 4 years,6 months,3 months,with an average age of 5.2 years.At the same time,20 normal children with the same age and gender were included as control group.Siemens 3.0TSkyra superconducting MR scanner was used to perform 3D T 1 mpragege 3D ASL sequence and routine scanning.40 cases of ASL data were analyzed by ASLTLBX software based on MATLAB.According to AAL template,inferior frontal gyrus,orbital inferior frontal gyrus,triangle inferior frontal gyrus,transverse temporal gyrus,hippocampus,cerebral island,posterior central gyrus,anterior cuneate lobe,supramarginal gyrus,central sulcus cover were extracted from the largest cross section of the smallest patient.The relative cerebral blood flow data were obtained from 13 brain regions of the temporal pole_middle temporal gyrus,the superior temporal gyrus and the superior temporal gyrus.The same region was extracted for 3 times,and the average value was obtained for each group in the case group and the control area.Using SPSS19.0 statistical analysis software,all the data were tested by Kolmogorov-Smirnov from normal distribution,and the measurement data were measured according to mean ±standard.The difference (?)±s indicated that all statistical tests were double-tailed probabilistic tests.Two-simple T test was used to compare the difference of relative cerebral blood flow between the affected and contralateral sides of region of interest and the left and right sides of the control group.In the control group,the perfusion asymmetry index of the region of interest brain area was compared with that of the control group by independent sample T test.The test level P < 0.05 was statistically significant.Results the hypoperfusion area was found in the hippocampus,and the changes of hypoperfusion in the precuneal lobe were statistically significant.The infra frontal gyrus,inferior trigonal gyrus,posterior central gyrus and temporo-polar middle temporal gyrus were higher than those in the contralateral side in the affected side of the orbit,the inferior frontal gyrus,the posterior central gyrus and the middle temporal gyrus.In the control group,there were significant differences in perfusion asymmetry between left and right sides of middle temporal gyrus,middle temporal gyrus and superior temporal gyrus.Although there was no significant difference in relative cerebral blood flow between the affected and contralateral sides in the case group,the AI value in the case group was still higher than that in the control group,and the possibility of low perfusion should be considered.Conclusion: In this study,non-invasive,non-ionizing radiation,simple and reproducible 3D-ASL technique was used to investigate the blood perfusion in the adjacent cerebral area of arachnoid cyst,and the hypoperfusion changes in hippocampus,anterior wedge lobe and cerebral island were found.At the same time,the perfusion of inferior frontal gyrus,inferior trigonal gyrus,posterior central gyrus and temporo-temporal middle gyrus were higher than that of contralateral,which reflected the compensatory mechanism of blood flow in the process of establishing dominant hemispheres.In addition to the cognition of cerebral perfusion in children with arachnoid cyst,there are also asymmetric changes of perfusion in both sides of brain of normal children,which lays a foundation for the evaluation of cognitive function of auxiliary nerves in the future. |
