Objective:To investigate the relationship between the duration of disease and ankle brachial index and provide risk assessment in patients with essential hypertension.Methods:180 patients with essential hypertension were divided into three groups randomly according to the duration of disease,the short duration group(time of onset≤5 years),the middle duration group(5 years<time of onset≤15 years),the long duration group(time of onset > 15 years).According to family self-testing blood pressure control,the long duration group was further divided into the two groups,stable blood pressure control group and unstable blood pressure control group.The ages,gender,duration of hypertension,ankle brachial indexes,body mass indexes(BMI)and serum biochemical parameters in these patients were collected.Risk factors of ankle brachial indexes were analyzed by single factor Logistic regression.Results:There were no significant differences among three course groups in gender,body mass index,FBG,TC,TG,LDL-C,HDL-C,UA(p>0.05).There was a significant difference among the three course groups in ages,ankle brachial indexes,and duration of disease(p<0.05).Single sample t test of the three groups’ ABI and the lower limit of normal shows than the short duration group ABI greater than 0.9(t=11.316,P < 0.001),the middle course of group ABI is greater than 0.9(t=2.826,p=0.006),long duration group is less than 0.9ABI(t=7.328,P < 0.001),have statistical significance.Ankle brachial indexes showed a negative correlation with course of disease(r=-0.634,p=0.000),age(r=-0.372,p=0.000)and total cholesterol(r=-0.186,p=0.012).with the older,longer duration of disease and higher total cholesterol,Ankle brachial index decreased.single factor Logistic regression show that the course of disease,age,gender are risk factors for abnormal ABI(p=0.000,OR=1.432;p=0.000,OR=1.106;p=0.036,OR=2.029).The incidence of ABI increased by1.432 times with the duration of disease increasing one years,also by 1.106 times with age increasing.abnormal indicators in women are 1.092 times more likely to occur than in men.There were no significant differences in long duration of disease groups in age,course of disease,BMI,FBG,TC,TG,LDL-C,HDL-C,UA(p>0.05)except gender(p<0.05).Conclusion:Ankle brachial indexes showed a negative correlation with course of disease.Ankle brachial index decrease with the duration of hypertension increasing.Whether blood pressure in patients with long-term essential hypertension control or not was not related to the degree of ankle brachial index which indicate that it is important to carry out arterial function testes in time.
